Responsibilities:

  • Reviews and assures that ongoing case coordination is designed to meet the comprehensive, person-centered needs of each resident including implementation and documentation of each residents’ treatment plan of support, admission, transition and discharge planning, and provides guidance for situations that arise with families and funding sources.
  • Develops and maintains collaborative relationships within and between all departments.
  • Provides leadership and direction to Prader-Willi Homes Case Coordinators, including assignment of caseloads, training, on-going supervision, and evaluations.
  • Active involvement with Family Engagement and Communication, including but not limited to Family Engagement Committee oversight. Develop and leads Case Coordination team as primary family/funder communicators.
  • Provides leadership and direction to Prader-Willi Homes Behavior Support Specialists, including assignment of caseloads, training, on-going supervision, and evaluations. Offers direction, input related to Behavior Health Supports and Therapeutic Interventions.
  • Provides leadership and supervision to the Administrative Assistant staff in the functions of maintaining resident records, maintaining safety and supervision plans, HIPAA Privacy, tracking and mailing of agency forms, and other administrative tasks.
  • Maintains department expertise and ensures compliance with program accreditation standards (COA), local, state and federal licensing and regulatory requirements, and community standards governing treatment services.
  • Attends workshops, seminars, in-services, etc.; participates as appropriate in various professional groups. Shares new information with department, leaders and employees.
  • Develops training standards for identified agency training and assists with conducting training.
  • Supports teams by providing ongoing training in behavioral management, including coaching, role modeling and giving technical assistance in developing behavioral interventions.
  • Ensures services are provided and documented in accordance with COA standards, funding sources, licensing agency requirements, and company policies/procedures.
  • Ensures that agency data is collected, reviewed and communicated to internal and external stakeholders as appropriate.
  • Serves as a member of the leadership team to evaluate program and agency functioning. Participation, as assigned, on applicable Company committees.
  • Performs human resource management and payroll processing functions for direct reports including interviewing and selection; orientation, training and performance monitoring and evaluations; coaching, counseling and corrective action and resolution of employee concerns. Monitors overtime. Maintains clear, objective documentation. Supports staff in their professional development. Develops and submits annual budget recommendations and manages budget.
  • Conducts self in positive, respectful and collaborative manner in accordance with the MyPath Beliefs and Behaviors demonstrating proactive actions and decision making to ensure supportive work relationships and a healthy and safe environment; is a role model to others. Communicates effectively verbally and in writing with Company leaders, external partners and employees, maintaining professional conduct and confidentiality.
  • Complies with and positively reinforces with others the expectations outlined in: Company Handbook; policies and procedures including emergency/disaster procedures, fire/tornado safety and maintains confidentiality.
  • Completes tasks as delegated by the Executive Director, as appropriate to the position.

Qualifications:

  • Bachelor’s Degree in Psychology, Social Work or other related field required.
  • Master’s Degree in Psychology, Social Work or other related field preferred.
  • Eight years experience with supporting children, adolescents/ and or young adults diagnosed with emotional, mental health, neurological and/or developmental disabilities required.
  • Five years of supervisory experience preferred.

*Locations include Oconomowoc, Watertown, Dousman, Hartland, Waukesha and Brookfield.*

Job Type: Full-time

Pay: $77,000.00 - $83,000.00 per year
