Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

The Service Coordinator is responsible for the professional assessment of the strengths, wants, and needs of persons with chronic and persistent mental illness, the development of individualized service plans with these identified individuals, and assistance with achieving these service plans goals within the framework of the Service Coordination rules, regulations, best practices, and SPA principles.

  • Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion.
  • Utilizes Sanctuary Models shared values, the Seven Commitments and tools to conduct daily tasks.
  • Assesses individuals strengths, wants and needs through a series of face-to-face, phone and collateral contacts using the principles of MH recovery.
  • Assists individuals in the development of individualized, strengths-based service plans after professional assessment has been completed.
  • Uses knowledge of Service Coordination rules, regulations, best practices and SPA principles to assist individuals with the attainment of service plan goals.
  • Uses professional judgment to determine the appropriate level of assistance necessary from the SC program to help individuals achieve their goals.
  • Acts as the single point of accountability, as defined under SPA principles, for all individuals assigned.
  • Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity and compassion.
  • Accurately and efficiently documents all work performed, including, but not limited to, progress notes, individualized service plans, strengths assessments, environmental matrixes, and other collateral documentation as necessary.
  • Abides by all state regulations.
  • Abides by all CCBHO Performance Standards.
  • Abides by all SPA principles.
  • Participates, as scheduled, in emergency on-call rotation which services all SC consumers.
  • Meets regularly with supervisor to report case summaries and all critical matters.
  • Meets all paperwork/computer work deadlines.
  • Meets or exceeds productivity requirements.

WAGES:$40,000 - $42,000annual
