At Ambassador Healthcare at College Park, you will plan, organize, and implement the overall operation of the Facility’s Social Services Department.
Responsibilities:

• The director will be responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings
• Direct, establish and plan overall policies and goals for the Social Services Department.
• Capable of resolving escalated issues arising from operations and requiring coordination with other departments.
• Assess patients regularly to make sure the facility continues to meet their needs and provide family with information about care planning.
• Involve residents and families in planning social service programs when possible.
• Provide solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required
• Meet with administration, medical and nursing staff, as well as other related departments
• Participate in Facility Surveys (inspections) made by authorized government agencies, as necessary or as may be directed
• Interpret social, psychological, and emotional needs of the resident, and family to the medical staff, attending physician, and other resident care team members.

Requirements:

• Must have 3+ years of experience working in a Skilled Nursing Facility
• Covid-19 vaccination is required
