Firwood Gardens is opening a 15 unit specialty needs program serving high acuity clients. This community will have leadership staff solely dedicated to serving this 15 person program, including 7 day a week nurse coverage and high staff to resident ratios on all shifts.

JOB SUMMARY::
A Social Services Director working with our Community Based Care facility will counsel, and aide residents and families requiring assistance. This position acts to meet those needs through individual and group social services programming. This position also promotes residents’ potential during their stay and collaborates needed healthcare resources.
RESPONSIBILITIES:
- Direct the overall operation of Social Services Department in accordance with federal, state and local standards, guidelines and regulations.
- Counsel's residents individually, or with family/support groups
- Refers residents to needed services such as Dental, Optometry, Audiology, Podiatry, Psychology, and Psychiatry and other community health resources
- The Social Services Director will compile records and prepare reports such as theft and loss
- Prepares timely documentation of social service interventions and assessments in resident's chart
- Coordinates with nursing to meet the residents’ optical, dental, and audiological needs and arranges transportation
- Attends resident care meetings
- Other duties as assigned
- Bachelor's Degree in Social Services or similar field
- Assisted Living/Memory Care experience preferred
