About the job:

The Social Work Case Manager routinely follows a case load of participants and supports the team with day-to-day participant needs in the implementation of the overall plan of care. Provides routine case management and psych/social interventions in coordination with the Social Worker to attain or maintain the highest practicable physical, mental and psycho-social well-being of each participant in alignment with their healthcare wishes and goals. While maintaining a highly functional work relationship with care team and other PACE staff, acts as an advocate for the participant to support a person-centered focus of care.


Salary: $55,000-$75,000

Schedule: Monday-Friday 8am-5pm

  • Bachelor’s or Master’s degree in Social Work or related degree in social sciences required.
  • Must be licensed where applicable by the State
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Ability to read, speak and write fluently in English
  • Ability and willingness to work in various locations, for planned activities or special events, family meetings and conferences, admission calls, admissions and tours.
  • Ability to work with participants, families, and others of varying levels of responsiveness and healthcare literacy
  • Familiarity with word processing, data processing, and computer entry skills to match existing clinical management and other software used by the program.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Preferred Skills:

  • A minimum of two years’ experience working with frail or elderly population required. Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable.
  • Excellent interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.
  • Must demonstrate ability to work with a diverse team and flourish in a flexible work environment.
  • Negotiation skills and ability to carry on several levels of reasoning at once during problem solving sessions; ability to think quickly and to write complex written arguments.

Essentials:

  • Acts as and engaged member of the Care Team. Regularly attends meetings and provides meaningful input.

  • In coordination with the MSW and Care Team Leader, is responsible for the evaluation of ongoing participant needs and the development of an effective plan of care that addresses the medical, physical, social, psychological, and spiritual needs of the participant in alignment with their healthcare wishes and goals.

  • In coordination with the MSW, responsible for the psychosocial wellbeing of participants. Provides ongoing support and education to participants and family regarding a variety of issues, including but not limited to: the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics and changing roles, PACE model and PACE health services.

  • Identifies, provides and/or coordinates for the medically-related social, psychological and spiritual needs of the participants. Implements social work interventions, case management, or psychosocial support as described by MSW. Coordinates closely with MSW and other team members any acute or complex cases and defers to MSW for any interventions/needs exceeding scope of practice.

  • As needed, may coordinate, schedule, and facilitate meetings with participants, family members/caregivers, and SCC staff.

  • Serves as a participant advocate to ensure all care is provided with a person-centered focus. Ensures that participant grievances are addressed and protects participants’ rights.

  • As needed and in coordination with Social Work Manager, engages in coordination with outside agencies such as Adult Protective Services, PACE Ombudsman, contracted providers and facilities, etc. in coordination of care and/or on behalf of the participant.

  • Assists with education to participants, families, staff, etc. on topics such as bill of rights, feedback and appeals process.

  • Identifies opportunities for quality improvement and engages organizational mechanisms as needed in coordination with Social Work Manager.

  • In coordination with Social Work Manager and other members of the Social Work Department, may design, implement, and/or facilitate participant and/or family support groups.

  • Is responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.

  • Manages and reviews participant’s records, charts, pertinent information and staff’s documentation. Documents as needed in medical records according to Senior CommUnity Care standards.

  • Educates staff in regards to care and treatment for the elderly as needed.

  • Follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.

  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.

  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

  • Performs other duties as required or requested.
