Responsibilities:
Responsible to assess, plan, implement, and evaluate the psychosocial services for patients of PrairieCare. The employee provides a comprehensive range of services to patients and their families, including assessment, treatment planning, care coordination/case management, and seeks to enhance the functioning of patients. This position may work with children, adolescents, and adults and serving patients with co-occurring disorders.
Responsibilities and Accountabilities:
- Accountable to have knowledge and understanding of Social Work practices and principles
- Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groups
- Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
- Responsible for communicating with patients and families to keep them involved in treatment plan decisions
- Coordinates care with outpatient providers upon admission and discharge
- Responsible for facilitating weekly treatment team meetings and creating master treatment plans/updating treatment plans as agreed upon by the treatment team
- Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
- Communicate clearly and effectively with clinical team members and Directors
- Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department
Hours: Monday - Friday 8:00am - 4:30pm
Qualifications:
- Social Work degree with liscensure LSW or LGSW or in the process of obtaining liscensure
- Experience in healthcare setting preferred
- Experience using electronic health records preferred
- Knowledge of Microsoft Office Products including Word and Outlook
- Able to effectively and efficiently communicate in written and verbal form
- Ability to smoothly and rapidly shift between dissimilar tasks
- Ability to communicate well both verbally and in writing
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
- Ability to make effective decisions while in stressful situations and circumstances
- After training, be able to manage disruptive and explosive patients.
