  • LICSW Needed for Palliative Care- Elliot Hospital, Manchester NH

  • POSITION SUMMARY
    • The Clinical Social Worker will provide assessments and therapy to patients and/or their families, provide group therapy and coordinate supportive services as needed in the community.
  • KEY RESPONSIBILITIES
    • Treatment Planning: Develop and implement treatment plans for patients and families. Focus on the patient illness, grief, and family bereavement process. Serves the best interests of the patient and exercises his or her best independent professional judgment when rendering care.
    • Counseling: Provides individual and/or family therapy to reach therapeutic goals.
    • Documentation of Services. Maintains ongoing documentation of patient interventions. Documentation must be completed within 24-hours of the service date. This includes documentation of initial psychiatric assessment, treatment plans, and therapy sessions and any other patient interactions including phone calls. Documentation will be completed using an Electronic Medical Record as provided by Elliot Health System. Cooperates with any audits of patient charts conducted to ensure compliance with all coding and documentation requirements and any compliance education reasonably required by the Medical Director.
    • Coordination of community resources and support for patients and families.
    • Interface with VNA and Hospice as needed based off patient condition and needs.
    • Liaison Functions: Represents the practice in the community. Participates in marketing, education, as well as other activities to educate providers and care givers on what Palliative Care is and how the practice is able to support patients through a disease process.
    • Participates in Quality Improvement Activities: Attends and provides input in Quality Improvement and Quality Assurance projects as deemed necessary by the Medical Director. Such Quality Improvement activities will include activities related to enhancing and maintaining patient satisfaction.
    • Supervision/Collaboration: Participates in ongoing peer supervision and collaboration to maintain licensure and to receive clinical input from peers. General oversight and clinical supervision will also be provided by the Medical Director or his designee.
    • Documents telephone conversation in the electronic medical record system provided by Elliot Health System.
    • Policies and Procedures: Adheres to all Elliot Health System polices and procedures.
    • Monthly Operations Review Meets with the Practice Manager
    • Credentialing: Submits credentialing and re-credentialing information to Medical Staff Affairs and Credentialing Specialist within seventy-two hours of receipt of applications.
    • Location of Services: Services will be provided at The Elliot Center for Palliative Care at 445 Cypress Street, Suite 8, Manchester, NH as well as in the home or hospital setting based upon patient need.
    • Other Duties: Performs similar or related duties as requested or directed. Participates in appropriate role in disaster, drills and other emergency situations, preparation of Joint Commission readiness, community events, committees and other related activities as requested.
  • REQUIRED KNOWLEDGE & SKILLS
    • Ability to plan and perform diversified duties requiring general knowledge of policies and procedures applicable within the area. Requires considerable judgment to work independently toward general results, devising methods, modifying or adapting standard procedures to meet different conditions, making decisions based on precedent and policy.
    • Ability to endure periods of heavy workload or excessive stress; ability to work with frequent interruptions and respond appropriately to unexpected situations.
    • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and coworkers. Ability to communicate effectively and establish a cooperative, collaborative working environment.
    • Ability to accept constructive feedback and initiate appropriate actions to correct situation.
    • General knowledge of normal and abnormal human development and behavior.
    • Must possess general knowledge of recognized treatment interventions and skills necessary to conduct individual and/or group therapy. This includes the ability to develop and maintain a therapeutic relationship with patients.
    • Must possess general knowledge of family systems and supports needed by family members. Must have the skills necessary to appropriately provide education and emotional support to family members, this includes communication skills.
    • Must possess skill in interviewing to gather data needed to diagnose the needs of individuals and their families.
    • Must have skill in preparing clear, concise case narratives and reports.
    • Must have knowledge and skill to refer patients to appropriate social services including State and Federal agencies.
    • Ability to work with mentally ill; character disordered; and/or cognitive impaired patients.
    • Ability to maintain effective working relationships with both professional and paraprofessional institution staff and public and private sector professional staff.
    • Ability to understand organizational systems and how to work within them for the benefit of the patient.
    • Ability to assess the level of dangerousness of patients and the potential for explosive behavior.
    • Ability to build and maintain effective working relationships with representatives of a wide range of community agencies.
    • Must possess the knowledge, skills and abilities to provide psychotherapy that is appropriate to the patient s age and intellectual development.
    • Must be able to demonstrate proficiency in assessing treatment and responses and adapting care to meet the need of the patient population served: children and/or adults.
  • EDUCATION/EXPERIENCE/LICENSURE
    • Education: Master of Science in Social Work required.
    • Experience: A minimum of one (1) year post-license experience, preferably in private practice, outpatient clinic or acute psychiatric environment. Basic knowledge of counseling interventions with experience utilizing such interventions.
    • Certification/Licensure: Licensed Clinical Social Worker (L.I.C.S.W.) required.
    • Software/Hardware: EPIC and Microsoft Office.
  • OTHER REQUIREMENTS
    • Schedule: Monday - Friday. Hours determined by provider and practice manager based off the needs of the clinic.
