Job description
JOB SUMMARY
Responsible for the evaluation, treatment planning, and treatment implementation of homebound patients. Provides physical therapy to patients of whom the majority are geriatric patients and are receiving services from Hamilton Medical Center Home Health in their home. The assignment of a Physical Therapist to a particular case must be made in accordance with a written plan of treatment established by a physician. CPR techniques and license. Knowledge of the professional Physical Therapy theory and practice to give and evaluate patient care. Knowledge of regulations. Skills in documenting clear/concise goals with objective measures and time frames. Skills in documenting client treatments with specific exercises, types of transfers, ROM, etc., all meeting state documentation requirements. Ability to document the patient’s treatment including type of exercises and number of repetitions so that another PT/PTA can duplicate client’s treatment for continuance of care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining effective working relationships with patients, medical staff and the public. Ability to maintain quality control standards. Ability to react calmly in an emergency situation. Ability to adapt and apply guidelines and procedures. Ability to communicate clearly. Ability to work independently in patients’ homes with direct supervision. Ability to drive self to various locations on a daily basis. Ability to supervise Physical Therapy Assistants and perform supervisory visits according to current professional standards and laws.
QUALIFICATIONS
Education: Graduate of an accredited college of physical therapy.
Licensure: Current Georgia licenses as a Registered Physical Therapist. Responsible for acquiring appropriate CEU’s as required by Georgia State Licensing Board for biennial renewal. Must have a valid driver’s license and automobile insurance coverage in accordance with Hamilton Health Care System Driver Requirements policy.
Experience: 1 year in clinical care setting or public health.
