Community Connections, Inc. is Idaho's premier agency serving adults with developmental disabilities, and we are seeking an experienced, motivated and caring Case Manager to work out of our Boise location's Supported Living Program.

We provide supports to our participants in their homes, and the Case Managers are a vital team member to ensure they can be as independent, safe, healthy and happy as possible. You would be responsible for supporting participant success while overseeing paperwork, collaborating with the guardian and team, and overseeing participant satisfaction and quality assurance. You will also work closely with the Supervisors as they support the direct care employees in the homes.

Case Manager must have experience working with individuals with developmental disabilities, and a four-year bachelor's degree in Psychology, Special Education, Sociology, Criminal Justice, Social Work, Health or another human service field; the experience and degree criteria is REQUIRED in order to apply. Case Managers must have current car insurance, your own reliable transportation, and able to pass a new Criminal History Background Check. Current CPR / 1st Aid and 'Assistance with Medications' certifications are also a plus (but not required to apply).
