Community Connections Inc. is looking for Supported Living Specialists to join our Supported Living Services team based in South Yarmouth, MA. In this role you will support individuals with disabilities by developing ISPs, managing case records, and completing health and safety documentation. You will partner with Community Connections as an advocate for program members to live independently and integrate into community life. We are looking for someone with a commitment to advocacy, promoting accessibility and serving the community.
What You'll Do:
- Develop and expand opportunities for community integration and participation to address personas served social and recreational interests
- Assist in completing the ISP process for DDS funded individuals
- Respond to emergency needs of individuals
- Oversee financial budgets, balance check books, bank deposit, timely payment of bills of persons on assigned caseload
- Attend meetings with various agencies to address the needs of individuals
- Ensure and safeguard CCI program members’ dignity, rights, and liberties.
What You'll Need:
- High School Diploma or General Education Degree (GED)
- Bachelor’s Degree in human service related field preferred
- 1+ years experience and/or training working with individuals with disabilities
- Valid driver's license, access to a motor vehicle, and satisfactory driving record
- Ability to work a flexible schedule
- Strong written and verbal communication skills
- Demonstrated ability to work independently
- Strong computer and word processing skills
