Position Summary
The Care Manager Surgical Coordinator performs the initial comprehensive assessment on elective surgical patients, assigned for Care Management support, in accordance with the Care Management Department policy. Assessment is focused on patient’s living arrangements, support systems, resources and current SDOH factors as it relates to discharge planning needs post-surgery. The CM Surgical Coordinator will collaborate regularly with surgical services, Surgical Navigation Center, post-acute care providers, community resources, patient identified care partners and Care Management leadership to support high quality and efficient delivery of patient care. The CM Surgical Coordinator will participate in initiatives that support the Orthopedic Committee, Geriatric Surgical Committee as well as other relevant projects related to the role. In addition, the CM Surgical Coordinator will also provide support to outpatient practices, as requested, for community resource education and referral for identified patients by outpatient providers.
Essential Functions and Responsibilities Includes the Following:
- Understands and adheres to the WPH Performance Standards, Policies and Behaviors.
- Completes comprehensive telephonic assessment of identified patients
- Collaboration and confirmation of plans with Care Partners as applicable
- Referrals for post-acute care facilities, follow up and confirmation of services prior to procedure
- Referral, ordering and confirming delivery of durable medical equipment
- Support with transportation needs prior to and following schedule procedures
- Education and referral of community resources, requested or identified, as a need for patient
- Provide relevant data and feedback to CM Leadership as it relates to initiatives impacted by role
- Performs all activities for multidisciplinary care coordination at the intra-hospital level of care and inter-hospital level of care.
- Performs all transitional planning activities including but not limited to patient/caregiver education, arrangement of all aftercare, and commercial payer involvement.
- Communicates and collaborates all relevant patient information to appropriate health team members.
- Ensures department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized.
- Documents per department requirements.
- Participates in Performance Improvement activities as needed
- Performs all other duties as assigned
Education & Experience Requirements
Registered Nurse or Licensed Social Worker
If Social Worker:
Master of Social Work Degree from a School of Social Work accredited by the Council of Social Work Education or BSN from accredited school of Nursing
Licensed Social Worker (LMSW/LCSW) approved by the State Education Department of the University of the State of New York or New York State RN license
If RN:
Current RN License in the state of New York
BSN required
5 years’ experience in an acute care hospital setting managing complex post discharge needs
Experience establishing and maintaining relationships with community agencies and partners key to providing optimal post discharge needs/care
NYS SCREEN Certification for SW or PRI screen certification for RN required.

Physical/Mental Demands/Requirements & Work Environment
- May be exposed to chemicals necessary to perform required tasks. Any hazardous chemicals the employee may be exposed to are listed in the hospital’s SDS (Safety Data Sheet) data base and may be accessed through the hospital’s Intranet site (Employee Tools/SDS Access). A copy of the SDS data base can also be found at the hospital switchboard, saved on a disc.
- Ability to remain in stationary position 90% of time
- Must be able to work in environment with continuous interruptions
- Demonstrates ability to analyze multidimensional problems into component parts and sort out tasks in priority oforder to solve the problem.

- Ability to use computer for extensive periods of time
- Ability to respond to frequent calls
- Constantly utilizes/operates computer to access and document information
- Ability to communicate with patients, families, and hospital employees on telephone and using email. Must be able to exchange accurate information in these situations.
