Job Summary:

The LPN at Thome PACE works in a close collaborative relationship with the Providers and Registered Nurses to provide integrative care to frail elders with complex needs. The LPN utilizes a systematic approach to practice incorporating critical thinking, planning, implementation and evaluation of outcomes to pro-actively manage and advocate for PACE participants. The focus of care provided is to enhance functional capacity, encourage autonomy, and assure coordination of care and services.

S kills and Knowledge
  • Must be self-directed, flexible and committed to the team concept.
  • Able to read, write, speak, and understand the English language.
  • Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations

Required Qualification:
• CPR Certification
• State of Michigan driver's license and maintain an acceptable driving record
• Reliable transportation
• Minimum of 1-year experience with the frail and elderly or long-term care population.
• Current MI Licensed Practical Nurse license
