Van Duyn Center is seeking a Transition Care Coordinator!

Experience in discharging in LTC required.

Position Summary:

This position is part of the social work department on VanDuyn’s premier rehabilitation unit. This position facilitates the transition from the acute care setting to rehabilitation to home. The Transitional Care Coordinator works with the interdisciplinary team, including the family and support systems for successful discharges. The transitional care coordinator develops and modifies the resident’s care plan and identifies any barriers to discharge. Sound discharge knowledge base, required. Experience and knowledge of post-acute transitions and home health preferred. Knowledge of Medicare, Medicaid and commercial payer regulations, useful.

Duties/Responsibilities:

• Discharge planning

• Coordinate care at home, as needed (CHHA, DME, etc.)

• Maintain current and accurate documentation

• Provide support and education to resident and their loved ones

• Conduct cognitive and psychosocial well beings assessments

• Create and oversee discharge summary completion

• Create care plans

• Assist with the completion of advanced directives

• Conduct interdisciplinary meetings

• Advocate for residents and ensure dignity is intact

Requirements/Qualifications:

• Bachelor’s Degree in Social Work or a related field

• Energetic, out-going personality

• Ability to work in a fast-paced environment

• Team player

• Ability to manage multiple tasks and meet deadlines

• Previous experience is required.
