Responsibilities

• Help patients by supervising care and treatments, leading patient assessments, creating personalized plans of care, targeting patient concerns and offering the best solutions.
• Resolve or report on patients’ needs or problems
• Monitor and record patient’s condition and document provided care services
• Work within and cooperate with a multidisciplinary team
• View each patient with a strong sense of empathy, listen closely to their personal stories, and get to know each one’s physical, mental and emotional needs.
• See patients progress due to the effort you and your team put forth each day and build rewarding relationships with them
• Spend each day helping someone to regain control of their lives with the goal of being discharged from the facility and back to their community, families and friends.
• Follow care regulations and standards

Skills

• Proven nursing experience
• Familiarity with professional and technical emerging knowledge
• Problem solving skills and ability to multi-task
• Compassionate with good communication skills
• Excellent teamwork skills

Credentials:

• Current LVN/RN licensures appropriate to state regulations required.
• CPR certification required
• One or more years of experience in an SNF setting preferred.
