ESSENTIAL DUTIES & RESPONSIBILITIES: A Van Driver/Float Life Skills Trainer provides direct support and must be able to perform all essential functions of the position consistently and successfully.

1. Ensure that individuals are offered personal choice and are supported in making their own decisions to the greatest extent possible.

2. Ensure that individuals participate in their community to the greatest extent possible in the most integrated setting appropriate to their needs.

3. Provides training in activities of community recreation and leisure, money management, personal responsibility, personal care needs and social interaction.

4. Safely transport individuals to/from various community sites and activities.

5. Follows through on any behavior management plans as established for individuals.

6. Completes and maintains accurate documentation relating to the numerous services provided to individuals and the progress they have achieved.

7. Communicates individual needs to The Supervising Life Skills Trainer.

8. Complies with agency incident reporting policies and procedures.

9. Maintains confidentiality of individual information in accordance with the requirements of NYS Mental Hygiene law and the Agency’s HIPAA Privacy Policies and Procedures.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of position skill and/or ability required.

  • EDUCATION and/or EXPERIENCE - High School Diploma AND one-year combined work, volunteer, internship experience with individuals with disabilities required. Experience working with individuals with severe behavioral challenges is preferred.
  • CERTIFICATES, LICENSES, REGISTRATIONS Etc. - A valid Driver’s License .

Job Type: Full-time

Pay: $19.00 per hour
