POSITION TITLE: Wellness Programs Manager
CLASSIFICATION: Regular full-time, Exempt
SUPERVISOR: Clinic Director
JOB SUMMARY:
Improve the overall health of the Greater Spokane community and expand the availability of health care by providing supervision and management of the Prevention Services programs. The Prevention Services unit plays a vital role in our community presence as a public health and wellness provider. This role will be responsible for managing some of the largest outreach and education programs for The NATIVE Project, including the Diabetes program; Employee Wellness; the Children and Youth Services programs such as Leadership Camp, Indian Youth Summer Program, Community Wellness, and, some Care Coordinator programs.
ESSENTIAL DUTIES/RESPONSIBILITIES:
- Oversees the planning, implementation, and evaluation of the NP Prevention and Wellness Programs.
- Plans and coordinates the activities of the Children and Youth Services programs.
- Liaise between the NATIVE Project and other community health partners.
- Meets directly with patients and community members to identify educational needs; provides individual instruction, group education, and clinics; utilizes Motivational Interviewing, cultural activities, and informational handouts/booklets, or partners with community resources as needed for a variety of Prevention/Wellness topics.
- Manages all Indian Child Welfare related activities.
- Regularly contracts with or maintains nutritionist visits documentation in the Diabetes Registry and Greenway Intergy for diabetic and other patients needing nutrition consults.
- Participates in various patient program committees, outreach events, and Health Promotion and Disease Prevention (HPDP) initiatives.
- Participates in community needs analyses and recommends ways to expand and improve outreach and HPDP programs through patient satisfaction surveys, focus groups, and collaborations with other agencies and peers.
- Participates in quality improvement of the program in conjunction with other services NATIVE offers.
- Oversee and manage NATIVE Project Prevention/Wellness department and staff.
- Assists in writing, managing and implementing all applicable grants and comply with all reporting requirements.
- Maintain patient records containing pertinent, accurate, and current information in compliance with Privacy Act, HIPAA, and RCW/WAC/CFR policy, procedures, and rules.
- Uses “Sacred Hospitality” in serving patients and peers.
- Possesses critical working knowledge of current Evidence-Based Practices and supports recovery-based lifestyles and patient outcomes.
- Can successfully pass a criminal background check, OIG check, and pre-employment UA/BA and show proof of required vaccinations.
QUALIFICATIONS:
- Excellent oral and written communication skills are required.
- Good computer skills are required, including experience using Electronic Health records.
- Proficiency with grant writing and grant management, evaluation, and reporting.
- Demonstrates personal and professional skills in the work related topics and the areas of cultural competence and emotional intelligence.
EDUCATION AND EXPERIENCE:
- Bachelor’s degree in Public Health, Education, Social Services or related field required.
- Prefer candidates with R.N., R.D. Registration, education or social work credential and be eligible for a Washington State certification.
- Prefer a Certified Diabetes Educator credential.
- Prior chronic disease and/or diabetes nutrition counseling experience as well as community health and/or prevention work experience.
- Experience working with children, youth, adolescents and their families.
- Experience working with diverse populations.
PHYSICAL DEMANDS: Must be able to move around the facility between1/3rd and 2/3rd of the day as well as sit at a workstation. Lifting up to 10 lbs. occurs half the time, while up to and above 25 lbs. occurs occasionally. Must be able to occasionally work evenings or weekends.
