Job Summary

Responsible for providing trauma informed care services as behavioral health consultant to medical providers at infectious disease clinic treating people living with HIV. Provides direct behavioral health interventions, in keeping with a trauma informed care philosophy, to patients with mental health and/or substance use disorders and with challenges in meeting HIV treatment goals in order to improve patient outcomes and satisfaction. Plans and implements short-term behavioral interventions to meet patient needs, advises medical providers on longer-term care, and refers patients needing higher levels of behavioral health care to psychiatric services. Assures compliance with policies and procedures, regulatory requirements regarding confidentiality, safety, and quality standards.

Essential Functions

  • Consultation with primary care physicians to enhance understanding of the patient utilizing a trauma informed care lens, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans. Provide consultation to and coordination of care on behalf of patients with primary care staff, identifying, referring, and advocating for patients needing behavioral health services as well as HIV transmission risk reduction planning and counseling.
  • Provide crisis intervention on behalf of patients with immediate availability when requested by medical providers, case managers, or patients.
  • Provide substance use/abuse evaluation, identification of maladaptive coping, and development of harm reduction strategies. Provide motivational Interviewing to engage patient in services aimed at symptom reduction. Facilitate / co-facilitate support groups.
  • Prepare and maintain patient charting and brief progress notes accurately and in a timely fashion as required by clinic and funder.
  • Participate in staff development activities as part of the clinic staff and as coordinated by BHC Coordinator. Attend clinic team meetings and supervision with supervisor (LCSW) to assure quality of patient care and adherence to clinic protocols.
  • Participate in Partnership Committees as requested by supervisor.
  • liaison to our consulting psychiatrists; and OP psychiatry department; participated in monthly committees such as pain management CQI subgroup, staff wellness and trauma informed care.
  • Other duties as assigned by supervisor.

Required Qualifications

  • Minimum of a Masters degree in Healthcare, HIV, mental health and/or substance abuse disorders or Equivalent
  • Minimum of 2 years of experience in a related field.
  • Licensure to practice in Pennsylvania required. (LCSW for social work).

Preferred Qualifications

  • LPC or M.S.W. form an Accredited Program with clinical training and experience in working with patients in medical settings.
  • Licensure to practice in Pennsylvania required Skilled in providing trauma informed cognitive, behavioral, and psychotherapeutic brief interventions to individuals, families, couples, and groups with behavioral health/substance abuse problems in a primary care setting.
  • Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
  • Ability to assist individuals, couples, and families with depression, anxiety, adjustment disorders, and other mental health diagnoses in outpatient setting. Familiarity with standard methods for assessing psychological and psychiatric disorders.
  • Experienced in working with patients with chronic medical conditions, working with inner city populations, working with patients from minority racial, ethnic, and sexual groups. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
  • Ability to assist individuals, couples, and families with depression, anxiety, adjustment disorders, and other mental health diagnoses in an outpatient setting. Familiarity with standard methods for assessing psychological and psychiatric disorders.
  • Experienced in working with patients with chronic medical conditions. Familiarity with the electronic medical record (EPIC).
