Mindoula is seeking a Program Manager for our Population Health Programs to lead a team of Case Managers in Nevada. Together, you will work on implementing Population Health Programs that deliver empowering outcomes to members in order to help them improve their own healthcare through personalized case management that targets each member's specific needs. Our Case Managers empower members to actively participate in improving their own healthcare through personalized case management that targets each member's specific needs. An innovative Program Manager seeks creative solutions to support Case Managers and Population Health initiatives, aligning with program goals and driving toward the desired outcomes. As the leader of a community-based team, they model a hands-on approach, empowering the team to meet members where they are and tapping into the wide range of local resources to deliver whole-person care.

How you'll lead:

Act as a player/coach, working alongside Care Extenders/Case Managers to build skills in everyday situations and to directly support members in more challenging situations or as needs arise, such as:

  • Create service plans using all available information about the member, including screenings, conversations, data, and knowledge of community resources.
  • Request consultation and diagnostic reports from network specialists.
  • Report critical incidents and information regarding the quality-of-care issues.
  • Build communication strategies to engage and build relationships with the member cohort.
  • Develop and coordinate community resource guides with an emphasis on medical health, behavioral health, violence prevention, and social services.
  • Lead our Care Management team to ensure member needs are assessed adequately and that all members have an appropriate care plan.
  • Ensure the team's schedules meet program needs.
  • Participate in the screening, interviewing, and hiring of new team members. Provide orientation for new team members, conduct performance reviews, and continuously coach the team to build skills and improve performance.
  • Analyze team performance to identify gaps and challenges and create plans to improve performance as needed.

What will make you successful:

  • A Master's Degree with Nevada licensure is REQUIRED: LCPC, LMFT, LCSW, PhD, PsyD, RN or equivalent.
  • Previous experience working with people who may have experienced interpersonal/intimate partner violence, sexual assault, human trafficking, and/or other vulnerable populations, and/or homeless, mentally ill, substance-using, or dually-diagnosed people is preferred.
  • Understanding of local domestic violence resources.
  • Experience in crisis intervention for youth and adults.
  • Ability to travel within the assigned region to meet with team and members as needed.
  • Experience in Care Management, preferably in Behavioral Health.
