SUMMARY STATEMENT & PURPOSE
The Patient Care Clinician (PCC) is primarily responsible for meeting with all eligible patients to discuss the benefits of IOP services as part of their continuum of care treatment and ensuring the proper referral is made. The PCC should have a clear understanding of the levels of services available in the outpatient program and understand the inpatient workflow process as it pertains to case management and discharge planning. The PCC supports the IOP Program by evaluating patient eligibility, completing clinical admission assessments, and completing the intake process for new patients into the IOP Program at INBH.
All employees are expected to be pleasant, respectful, and courteous in all interactions with patients, families, staff, and visitors of the Hospital, as well as being a positive representative of INBH at all times.
MINIMUM QUALIFICATIONS
· Bachelor's Degree from an accredited college or university in social work, psychology, mental health, or a related field
· At least one (2) years of related experience providing customer service in a medical setting.
· Agency Affiliated Counselor
· BLS, First Aid, Handle with Care and Verbal De-Escalation certification, provided by INBH.
PREFERRED QUALIFICATIONS
· Two years working experience in a hospital, behavioral health, or medical setting.

SPECIFIC PERFORMANCE RESPONSIBILITIES
All functions are essential functions unless otherwise noted.
The job functions of this position are not limited to the duties listed below.
1. Manages the workflow of the step-down process
2. Meets routinely with all eligible patients during their inpatient treatment episode to assess aftercare needs and potential barriers to outpatient engagement and introduce to outpatient programs
3. Collaborates with inpatient social services, utilization review, and business office to determine patient eligibility for outpatient programs
4. Provides education and support about the significant recovery benefits for continuing care post-discharge from inpatient
5. Meets with patient on first day of PHP/IOP programming to support connection to the program and overall orientation and completes required assessment and safety documentation
6. Identifies and reports trends including Stepdown Report to continue program expansion and reduce barriers to treatment
7. Collects, enters, and maintains data relevant to patient referral and admission to outpatient programs and generates reports of the data as requested
8. Completes outpatient admission paperwork and assessments (as applicable) and admission into MS4
9. Files, assembles new patient charts, collects collateral for charting, and completes clerical needs of new patient intake process on an ongoing basis
10. Facilitates IOP groups and documents group attendance and participation on group note on an as-needed basis
11. Works closely with Business Development implementing strategies and opportunities to promote growth of the IOP program.

· Ability to lift and carry up to 25 pounds.
· Ability to reach above and below the waist, turn, twist and manipulate small tools (copier, computer, telephone, calculator, safe, fax machine).
· Ability to move frequently in the completion of job requirements.
· Ability to handle a variety of repetitive tasks.
· Ability to handle multiple projects and tasks under deadlines and with short notice.

Pay: $23.15 - $34.73 per hour
