Position Title: Care Coordinator Job Status: Full Time, Exempt
Reports To: Executive Director
POSITION SUMMARY
The Care Coordinator provides a range of supportive services and referrals to family members to ensure the safety of children and the well-being of family members.
Responsibilities include identifying services for children, teens, and adults in individual and group settings; providing a range of child building and parental life skill information and activities. Assists with families basic needs and more.
ESSENTIAL JOB FUNCTIONS
- Perform Child Safety Assessment and evaluation of family dynamics and service needs
- Conduct intake process and create an appropriate Family Success Plan with each client
- Provide appropriate psycho-‐educational intervention for clients and families, including individual, parental , family and group counseling
- Provide a range of child building and parental life skill information activities
- Refer, and link, to outside agencies and private providers as appropriate
- Assess the need for services and form links with appropriate resources
- Prepare materials or information packets for clients, as appropriate
- Collaborate with DHS-‐CWS intake workers and other community agencies with which families are involved
- Assist with program planning, development and evaluation
- Log, and maintain, all required client information and documents into database
- Maintain professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in the field
- Comply with the guiding values, vision, mission and goals of NPK
- Relate with clients, public and co-‐workers in a mature and professional manner
- Participate in program and organizational quality management activities
OTHER JOB FUNCTIONS
- Attend all NPK staff meetings and trainings as requested
- Perform all duties with a high degree of accuracy, proficiency and with a minimum of supervision
- Perform other duties as assigned or as time allows
MINIMUM QUALIFICATIONS
Education
Bachelor’s Degree in Social Work preferred but experience considered.
Experience
One year experience working with West Hawaii Social Service Agencies preferred
Skills and Knowledge
- Knowledge of best practices in the prevention of child abuse and neglect
- Ability to provide child building and parental life skills information and activity
- Ability to anticipate tasks and prioritize assignments
- Ability to interview clients in non-‐threatening manner
- Ability to maintain highly sensitive, confidential information
- Knowledge of social services and health care delivery system
- Knowledge of word processing, spread-‐sheet and database software
- Excellent oral and written communication skills
- Ability to care and service to client age and needs
- Sensitive to cultural values of client/family
Other Requirements
Valid Hawaii driver’s license and proof of automobile insurance
Reliable transportation
Regular travel to meetings, events and client visits.
Satisfactorily complete Criminal and DHS-‐CWS background check
Acceptable Hawaii Traffic Abstract
Kona/West Hawaii resident preferred
WORKING CONDITIONS
Generally good working conditions with little or no exposure to extremes of heat, noise or health hazards, varying and unpredictable situations,
Handles emergency or crisis situations professionally and appropriately
Occasionally required to work irregular hours
Travel throughout West Hawaii required
PHYSICAL DEMANDS
Light physical lifting /carrying 30 pounds
Occasional standing, walking, reaching, bending and kneeling
