Behavioral Health Care Management Specialist (Temporary)

The Care Management Specialist for Behavioral Health is responsible for responding to requests for members receiving, or wanting to receive, behavioral health care by linking the member to appropriate providers.
Coordinate care across the members care team, including HPSM’s ICM team, to ensure that appropriate care has been arranged and making sure the member and his/her caregivers understand the follow-up plan of care.
Position overview
- Independently handle requests for provider matching, assessing the request, the member’s needs, and facilitating appropriate interventions and follow up.
- Maintain clear understanding of HPSM and County Mental Health Plan Benefits and Systems navigation for Medi-Cal Members.
- Communicate and coordinate with referring and treating providers and refer them to additional provider training as needed.
- Assist members with benefit navigation.
- Work with healthcare providers and members to address any barriers to care.
- Perform data entry of medical information; maintain tracking and reports.
- Share knowledge with Integrated Care management team and Customer Supports teams with information on the Mental Health Benefit as it relates to individual members needs.
- Cross trained with the Integrated Care management team for broad awareness of benefit, resources and supports available to HPSM members.
- Serves as a member liaison, providing assistance to HPSM members with Behavioral Health Treatment needs with in the HPSM Benefit and County mental health plan.
- Actively participate in team meetings.
Requirements

Education and experience
- Bachelor’s Degree in nursing or related Health/Social Services field preferred.
- 2 years of experience in mental health and/or substance abuse treatment environment.
- Experience working with the health needs of the population served.
- Experience in Care Coordination.
Knowledge of:
- Strong knowledge of the complexities of working with the elderly and disabled population.
- Awareness of Community and Mental Health resources.
- Comprehensive knowledge of Care Coordination.
Skills:
- Personal computers and proficiency in Microsoft Office Suite applications, including Outlook, Word, Excel, Access and PowerPoint.
- Bilingual skills highly preferred, particularly Spanish, Tagalog or Chinese.
- Demonstrated skills in managing multiple priorities and staying organized using multiple systems.
- Communicate effectively, both verbally and in writing.
Ability to:
- Ability to learn the benefits available through Medi-Cal and Medicare.
- Work cooperatively with others.
- Work as part of a team and support team decisions.
- Adapt to changes in requirements/priorities for daily and specialized tasks
Salary
The starting salary range depends on the candidate’s work experience.
This Temporary position qualifies for 24 hours of sick time per year.
