Case Manager/Service Coordinator (SSA)
(This is not a management or counseling job)
Qualifications: Successful candidates
- Must have a Bachelor’s Degree in Human Services, Special Education or another related field
- Must have or be eligible to obtain Ohio DODD SSA certification
- Must be highly self-motivated, self-disciplined, and able to work cooperatively with others
- Must have strong communication skills and leadership qualities
- Must be organized, accurate and have the ability to remain professional and compassionate in stressful situations.
- Most possess computer skills
- Must be willing to undergo criminal background checks, work reference checks and a drug screening.
- Preferred two years of experience with individuals with developmental disabilities
- Preferred experience in mental health field
Job Summary: Successful applicants will
- Work one-on-one with individuals with developmental disabilities and their families, both virtually and in-person
- Coordinate services and supports using a person-centered approach
- Assist with the development and revision of individual plans and budgets
- Provide emergency assistance both during and outside of regular working hours; serve as part of an on-call rotation
- Communicate the availability of resources both within the agency and in the community
- Monitor service provision for quality, continuity, and effectiveness
- Be an advocate for individuals' choices and rights
- Complete required documentation, maintain confidentiality
- Attend and lead meetings
- Work in conditions that may risk exposure to blood- and air-borne pathogens and/or aggressive behavior
Hours: 40 hour per week schedule requiring some flexibility, working from home and occasional on-call for weekends, evenings, and holidays.
Salary & Benefits: Pay:Commensurate with experience and relevant education. Full benefits package including health, vision & dental insurance, life insurance, paid vacation, sick leave, and personal leave, enrollment in Ohio Public Employees Retirement System, continuing education reimbursement, and IRS mileage rate reimbursement.
