Mountain Community Health Partnership (MCHP) is seeking a full time Certified Nursing Assistant (CNA). The CNA is responsible for providing clinical, laboratory, and business office assistance in the front office and clinical setting under the supervision of the site manager (for clerical responsibilities), or physicians, mid-level practitioners (NP, PA, CNM), or nurse manager (for clinical responsibilities). The CNA is a member of a care team lead by the healthcare provider and serves as a liaison between the patient and the healthcare provider for establishing and maintaining quality patient care.

Clerical duties include but are not limited too welcoming patients by greeting them, in person or on the telephone; Answering or referring inquiries; Verifies patient information by interviewing patient; reviewing and/or recording demographics, type of insurance, purpose of visit, and assisting patient to complete other pertinent documents such as HIPAA forms, review of systems (ROS), surveys, etc; Obtains co-pays, posts them to patient’s account, and prints/provides receipt; Completes ancillary and referral forms as needed; Schedules appointments and maintains providers’ schedules in the EMR; Maintaining and filing/scanning medical records; Demonstrates accuracy and efficiency in using the EMR; Maintains patient confidentiality and confidentiality of medical records; Is cross-trained in all duties of Check-in Clerk, Medical Records Clerk, Referral Clerk; Performs other appropriate duties as assigned by the Office Manager.

Clinical duties include but are not limited too demonstrating accuracy and efficiency in data collection using the EMR; Facilitation coordination of care by obtaining and tracking test results and consult records; Communicating with healthcare providers, community organizations, health plans and facilities and specialist; Compares data collected to normal values and findings; Engages in pre-planning for all patients; Engages in daily "huddling"; Providers ongoing assessment of patient status for changes in condition, position or negative; Recognizes and reports immediate need for intervention to nurses, physicians and mid-level providers; Carries out medical providers' orders accurately, safely and efficiently; Participates in teaching patient/family under the supervision of RN, physician, mid-level practitioner, other duties as assigned by the Nurse Manager or Office Manager.

Must obtain a valid Nurse Assistant Certification. Prior Experience as a CNA in clinical setting, preferred. Current BLS certification, preferred.
