Exciting NEW opportunity exists for a Client Navigator to become a key member of our Same Day Access program! We are looking for a compassionate and professional team member to guide clients through the Same Day Access behavioral healthcare appointment, process, and continued follow-up.

  • Manage the client’s visit to Aspire from the intake process, through the visit with the clinician and through the care process
  • Respecting the client's choice, explain and collect informed consent for treatment and participating in the grant funded activities, and ensure that clients are fully informed of their legal rights
  • Collect and accurately document intake assessments prior to the clinician visit
  • Manage several clients each day at different points in their initial visits and treatment plans
  • Support client’s non-clinical needs with deep understanding of both Aspire and community resources to remove barriers to the client’s recovery, including addressing social determinants of health
  • Participation in Daily Care Huddle to help determine best course of action for each client
  • Takes vital signs and weight of client and record in electronic health record

  • Ability to assess situations and use appropriate judgment to determine most appropriate course of action
  • Time management and ability to successfully organize priorities
  • Willingness and ability to support a diverse population of patients


Required Experience
  • Knowledge of the health care system and experience or ability to work with busy clinicians to help manage a successful client visit
  • Prior experience in researching, and knowledge of community services available for clients
  • Prior experience in taking vitals preferred or willing to be trained.
  • Bachelor's degree
  • Community Health Worker (CHW) Certification and/or a bachelor’s degree in a healthcare-related field is preferred
