Function: The in-patient Residential Case Manager (CM) serves as a participating Member of the clinical team and provides services to promote positive behavioral change for detoxing and residential clients. The residential/detox case manager will also work closely with the medical team to assist in providing the needs for the all BRH clients. The CM provides support and encouragement to help clients meet their recovery goals. Case Managers also provide or coordinate all covered services (e.g. case coordination, outreach, crisis intervention, housing, and counseling) to address clients’ individual treatment needs.

Essential Duties and Responsibilities:
Provide holistic and comprehensive case management services to all clients including: intake assessment, benefit assessment, goal setting, long-term care plan development, weekly case plan development, progress monitoring, individual money management, tenant education, advocacy and referrals for all NMRE detox clients and residential clients that have been referred by their primary therapist. Maintain a caseload of BRH clients and families.
Meet with clients weekly for their entirety of their stay in detox and residential services. After they have discharged the case manager will check in with client by phone if they have signed a consent for contact. We do not currently do follow-up face to face meetings.
Provide all required documentation in a timely manner, which may include client follow-up, case notes, outcome evaluation, client contact sheets, and evaluations.
The position is responsible, but not exclusive, for the following:
- Coordinates the delivery of integrated care services to Clients, in collaboration with clinical and other staff to address the Client’s needs.
- Performs integrated recovery support assessment, health care assessment and annual updates.
- Ability to work independently with strong sense of focus, task-oriented, nonjudgmental, open personal qualities, clear sense of boundaries.
- A strong sense of and respect for confidentiality involving both clients and fellow employees.
- Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate.
- Assists in development, implementation and revision of integrated service plans; assures that services provided are specified in the integrated service plan and monitors progress toward treatment goals, crisis plans and coordinating of recovery teams.
- Monitors level and intensity of services contained in the Clients’ individual integrated service plans.
- Assist and support Clients to follow through on treatment and/or medical interventions and diagnostic testing.
- Assesses and addresses barriers to health.
- Obtain records, notes, reports, and consultations from primary, specialty and other providers and ensure that the information is accurate and appropriately filed in the electronic health record.
- Good documentation skills. Excellent written and verbal communication skills, ability to establish rapport.
- Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
- May provide health literacy and health education groups.
- Offers guidance and education related to client’s individual needs.
- Connects Clients to resources within BRH and the community to address identified treatment concerns.
Review details and expectations about the referral with clients and or family Provides Client advocacy and attends treatment appointments, as needed.
- Assists with coordination of scheduling of appointments for Clients.
- Attends all staff meetings and all other required meetings.
Maintains Client records in accordance with agency and funding source requirements. Maintains regular contact with treatment staff to inform and be informed of Client’s progress and makes recommendations for continuing care.
- Performs other related duties in accordance with agency growth and changes.
- Filling out the proper forms in KIPU:
o Case Management Needs Assessment o Case Management Note – after every meeting
- Inform whomever is in charge of NMRE billing the client’s aftercare plan and case management.
- Inform therapist of any case management services provided so they can include in client’s discharge summary.
- Assist client with housing placement if needed (residential, transitional)
- Assist client with making follow up appointments with their primary care doctor, or assist in establishing a primary care doctor in their area.
- Staff meetings as needed.
- Follow up with Outpatient Services
- Any other additional resources as identified. (e.g. food stamps, Medicaid, utility assistance programs, etc.)
SKILLS / REQUIREMENTS
Education & Experience Requirements:
- Masters or Bachelor’s degree in a field related to Social Work and no Substance Use Treatment related experience OR;
- Masters or Bachelor’s degree in a non-healthcare related field and 1 year of full-time related Substance Use Treatment experience OR;
- Associates’ degree in a healthcare related field and 2 years of full-time Substance Use Treatment related experience OR;
- Associate’s degree in a non-behavioral or health related field and 2 years full-time Substance Use Treatment related experience OR;
Additional Requirements:
- Reliable Transportation REQUIRED
- Advanced knowledge of 12-step treatment model, integral recovery and its application in residential treatment.
- Specific knowledge helpful in addiction and treatment resources.
- Advanced assessment, treatment planning, discharge planning, and documentation skills.
- Knowledge of treatment and referral community in Northern Michigan and understanding of statewide resource database.
- Valid Michigan Driver’s license, proof of current insurance and willingness to use personal vehicle. Driver’s license is an essential requirement for performing duties for this position.
- Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years.
- Other duties will be assigned as need arises and according to programmatic need.
