We are currently seeking an experienced Clinical Nursing Team Trainer. This role is non-exempt (hourly) and would work Monday - Friday during the day with no weekend availability needed. This role will be responsible for training the Clinical Nursing Team (CNT) and staying proficient in all the roles within our Clinical Nursing Team.

Adult & Child Health currently operates four Federally Qualified Health Centers (FQHCs) with close to 50 staff working within those clinics. In 2020 alone, we had over 7,000 primary care visits.

What You'll Do

  • Provide orientation for CNT and Health Care Coordinator new hires
  • Monitor training modules within Relias to ensure completion by team members within the specified time
  • Educate staff/patient/family about diagnostic procedures, nutrition and maintenance of health and wellness
  • Adhere to infection control/safety guidelines and confidentiality policies
  • Coordinates the training of staff members to ensure they are knowledgeable in their role, policies, and procedures including quality controls
  • Collaborate with the nursing leadership team on educational needs of the team and complete follow up with documentation of training with team members
  • Collaborate with the A&C training team to ensure continuity across the A&C training team and the clinical nursing team for training needs, competency assessments or any additional program implementation or roll outs
  • Coordinate the scheduling of in-services when appropriate for new hires and reeducation of staff
  • Support the needs of the clinic by flexing to cover vacancies on the CNT schedule to maintain proficiency in the different roles
  • Works a minimum of once a week in either an integrated nurse role, healthcare coordinator, or MA role
  • Maintains/reviews patient records, charts and other pertinent information when working in any of the roles
  • Responds to/refers patient treatment-related phone calls
  • Professionally represents A & C in the community participating in building and instruction of educational curriculums for A&C or potential A&C clients
  • Document and coordinate data, information, and clinical practices across multiple EMR’s and databases
  • Perform complete assessment of medical and behavioral healthcare needs, family support structure, and patient eligibility status to obtain resources
  • Effectively communicates barriers to healthcare teams
  • Knowledgeable of Medicaid/Medicare guidelines, Marketplace plans, and commercial insurance benefits and services
  • Advocate for patients/clients by communicating patient desired healthcare choices

Requirements:
  • Minimum of a two-year nursing degree. Experienced LPN applicants accepted, RN preferred, but not necessary
  • Minimum of five years in a clinical setting
  • Knowledge of medical terminology and anatomy
  • Familiarity with population health, Chronic Care Management, Diabetes Education, and population management best practices
  • Familiarity with health care systems, regulations, policies, and functions
  • Knowledge of care management processes and best practices
  • Knowledge of medications and their effects on patients
  • Knowledge of infectious disease management and control and safety standards
  • Complete the American Diabetes Association training for Diabetes education certificate within the first ninety days of start date and maintains throughout employment without lapse
  • Complete BLS provider Instructor training/certification within one hundred eighty days of start date and maintains certification without lapse throughout employment

Equipment Operated: Phone, computer, EHR, Microsoft applications, Teams, Skype, and Ampernet

Work Environment: Medical exam and treatment rooms and medical offices. Environmental setting (lighting, temperature, air quality, acoustics) controlled.

Physical Requirements: Must possess the physical and mental abilities to perform the tasks normally associated with a CNT trainer such daily standing, squatting, walking, bending, and maneuvering. Occasional exposure to communicable diseases and biohazards. Stress can be triggered by workload and difficult patients.
