The Cognitive Community Health Specialist will be responsible for assessing and connecting children, youth, and their families who have been identified by the behavioral health clinical team to a continuum of coordinated mental health treatment and wellness services. The Cognitive Community Health Specialist will be responsible for completing a whole-person needs assessment that determines the child's, youths, or family member's social determinants of health and for developing a family-driven and youth- guided care plan that addresses needs across life domains. This role requires familiarity of community services because of the need for managing communication and coordinating services between agencies necessary to help children, youth, and their families get the services they need.
Examples of Work Performed:
- Establish and maintains working relationships with other tribal programs as well as non-tribal programs and resources that pertain to YNBHS patients and families.
- Reviews policies, or creates policies, and MOUs to create systematic changes that will improve the quality of services and will ensure provision of culturally appropriate services to meet grant requirements.
- Develop and maintain good working relations with all community agencies staff.
- Participates in Supervisory, Multi-Disciplinary Team Staffing, training, local Advisory/Coalition meetings, and child protective meetings.
- Maintain monthly statistics, complete evaluation criteria as required by contracts, and participate in meetings as needed.
- Continue to serve as Lead Care Coordinator responsibilities.
- Will be responsible for coaching, modeling and accompanying Care Coordinators and Peer Support Specialists to acclimate to their role as the Lead Care Coordinator.
- Will provide oversight of the case management for SOC Navigators, and Peer Support Specialist.
- Coordinates and provides training, and educational programs in conjunction with other organizations, other departments, and community partners that support person's success.
- Assist in development of the intake assessment, diagnosis, ongoing brief, and community support services. Including case management, to identify children, families, and adults in a culturally competent manner within the most appropriate environment. Must maintain a minimum of 25% work hours in billable services hours.
- Provide services, assessments, individual, family and group therapy for all required populations such as children, adolescents, adults, elderly, GBTLA, and veterans in a variety of settings including office, IHS clinic, local Hospitals, Tribal Court, Tribal Corrections, homes and some off reservation settings.
- Develops and implements treatment plans which focus on client strengths, needs, and goals within the briefest time frame for successful achievement.
- May be assigned new projects and tasks not included on the description, or to assist other Behavioral Health staff as requested by supervisor ensuring the efficient and effective functioning of the work unit.
Knowledge, Skills and Abilities:
- Knowledge of the basic principles, practices, and professional standards of providing clinical behavioral/mental health services to tribal communities.
- Knowledge of completing documentation not limited to treatment plans, assessments and service authorizations.
- Ability to complete billing in department's electronic health records software in a timely manner.
- Ability to make professional evaluations, decisions, and recommendation for treatment planning and implementation.
- Knowledge of community agencies and services provided.
- Knowledge of HIPPA laws and how it pertains to Mental Health policies.
- Ability to navigate service system and to ensure that children, youth, and their families are connected to services.
- Knowledge, skills, and ability to help Yakama children and families by providing services in ways that respect Yakama culture and spiritual practices.
- Skills in using computer software such as Excel and Microsoft word.
- Ability to provide services in the Yakama context of extended family and community involvement.
- Ability to handle multiple projects at the same time in the context of a quality improvement program.
- Ability to influence and facilitate community and group decision-making processes through knowledge, ideas, and persuasion.
Minimum Requirements:
- Master's Degree in Education, Social Work, Psychology, Nursing, and/or Related Counseling field, and 1 year of related clinical experience working with children, families and other individual's psychosis and/or other behavioral health issues.
- Must have or apply for associate/affiliate status through Washington State Department of Health. Full licensure must be acquired within the time frame established by the Department of Health and Director.
- Must maintain strict confidentiality.
- Required to pass a pre-employment drug test.
- Required to successfully pass a criminal background check.

TRAINEE: The Yakama Nation reserves the right to practice Indian Preference by selecting an Indian applicant as a trainee at a reduced pay rate than announced. A training plan is developed to assist employee to reach required qualification requirements and performance level. After completing training plan, work performance will be evaluated. Upon satisfying qualification requirements, employee will obtain entry level pay.
INDIAN PREFERENCE: In efforts to practice Self-Determination, the Yakama Nation grants Indian Preference for employment, promotions, and transfers to its employees and qualified applicants as appropriate. This may limit career opportunities for non-Indian applicants and candidates.
Preference for employment will be granted to qualified individuals meeting minimal qualification criteria in the following order pursuant to the YN Personnel Policy Manual Section 2.1.3:
- 10-points= Enrolled member of Yakama Nation.
- 8-points= Indian (enrolled) spouse of an enrolled Yakama member.
- 6-points= Other Indian (member of a federally recognized tribe).
- 8-points= Indian (enrolled) descendant of enrolled Yakama member.
- 2-points= Non-Indian spouse of an enrolled Yakama member.
- 0-points= Non-Indian.
Personnel Policy Manual Section 2.1.4:
- Positions funded under non-638 federal funding: 8-points= Indian (member of a federally recognized tribe).
VETERAN PREFERENCE: Is pursuant to Section 402 of the Vietnam Era Veteran's Readjustment Assistance Act of 1974. For verification, must provide a copy of honorable DD-214.
Application points using the weighted point factor system: Disabled Veterans receive 10 points; All other Veteran's receive 5 points.
