FULL-TIME COMMUNITY BASED CASE MANAGER - LANCASTER COUNTY, PA
Family Resource Specialist
SALARY RANGE - $36,000 - $42,000 PER YEAR
We are seeking an experienced and compassionate Community Based Case Manager to join our team. In this role, you will support families in our community by providing them with resources, advice, and assistance. You will be responsible for connecting families with local services, such as housing assistance, healthcare, and other support services. As a Community Based Case Manager, you will also provide guidance and direction to parents and families in need, helping them find the resources they need to create a better future for their children. To provide excellent service, our families are our priority so working with their schedules is key to creating a healthy rapport with them. Evenings and/or weekends may be required.

The qualified Community Based Case Manager will have:
  • Bachelor's degree or
  • Associate's Degree with relevant social work or human services experience..
  • Valid driver’s license, auto insurance and access to a vehicle.
  • Pass current, applicable clearances.
  • Pass pre-employment, post-offer drug screening.
  • Ability to work non-traditional hours including evenings and weekends.

The qualified Family Resource Specialist will:
  • Assess and engage families in service planning based on demonstrated needs.
  • Plan and implement appropriate interventions using evidence based and best practice approaches.
  • Maintain open communication with referring authorities.
  • Attend court hearings and provide testimony as required.
  • Submit accurate, required notes within 24 hours of service and complete reports within assigned timeframes.
  • Transport clients utilizing personal vehicle, as necessary.
