SUMMARY

This is a full-time exempt position that has dual roles. The position’s primary responsibility is to supervise a unit of employees who provide Community Health Services. In addition to the supervisory duties, may be responsible for providing direct services to clients while acting as a Community Health Worker. Primary work hours will be Monday through Friday, 8:30 am to 4:30 pm,with a 1/2-hour lunch (37.5/week) unless otherwise agreed upon.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Includes the following (other duties may be assigned as deemed necessary by management):

Program Development and Planning:

In conjunction with the RHIS Leadership, this position is responsible for the development of policies, procedures, and reporting related to the development of the Community Health Services program. Assures compliance with all standards established with entities such as Southwest Behavioral Health Management, Westmoreland County BHDS, and Pennsylvania entities such as, the Office of Mental Health and Substance Abuse Services andPa Certification Board (PCB).

Program Supervision and Coordination:

Community Health Supervisor/Worker is responsible for the day-to-day supervision, coordination, and implementation of program activities for the purposes of efficient and effective service delivery. They monitor the compiling, maintenance, and review of necessary client records to assure compliance with RIHS and county and state policies. Below is a list of other supervisory duties:

· Review and approve timesheets and expenses.

· Conduct interviews and hiring of employees.

· Complete evaluations and assess employees’ performance, which include identifying employees’ strengths and weaknesses.

· Problem solves and address employee issues, hold employees accountable to the expectations of the program and uphold agency’s policies and procedures.

· Review and approve requests that are submitted through the reinvestment funds. Ensure monthly and yearly funding requests are tracked and reported.

· Conduct supervisions to provide feedback to employees.

· Provide necessary outcome and fiscal reports to RHIS leadership, County BH/DS office, and Southwest Behavioral Health Management.

· Establish and monitor program and SDoH Commodities standards within RIHS and providers in which RIHS enters agreements to distribute SDoH commodities funds. Ensure the preparation and maintenance of statistics and reporting.

· Provide leadership, direction, and training of staff in reference to program development to meet client and agency needs.

Other Responsibilities:

The Community Health Supervisor/Worker will be responsible for providing Community Health Worker services to eligible individuals whenever the other Community Health Worker caseloads are at full capacity. This includes, but is not limited to, conducting standardized Social Determinates of Health (SDoH) interviews and screenings. Prioritizing and meeting the SDoH needs of the client by advocating and empowering the individuals they serve, completing referrals for other community services, and maintaining proper communication with these providers. Maintaining daily documentation of case activity, electronic records, and proper follow-up to ensure the referrals/linkages/warm handoffs were successful and effective. This position may be required to perform other duties as assigned by the supervisor or management staff.

Crisis Intervention:

Community Health Supervisor/Worker must have the ability to identify and defuse individuals who are in crisis as a result of not having their Social Determinants of Health needs met.

Professional/Technical Responsibilities:

Community Health Supervisor/Worker must have extensive knowledge of theories, principles, and techniques related to service provided. They need to have the ability to establish/maintain effective working relationships with clients, associates, and agency personnel. A knowledge of community resources and the capacity to facilitate coordination activities and an ability to prepare clear, concise reports and to communicate effectively, both orally and in writing, is necessary.

Compliance:
It is the expectation that employees will adhere to any/all RIHS, county, state, and federal guidelines, regulations, policies, and laws governing the Funding Coordinator program. Employees of the Community Health Supervisor/Worker program will be expected to attend specially designed training programs provided through RIHS, Managed Care Organizations, county, state, and federal entities that are relevant to the Community Based Care Management Initiative.

QUALIFICATIONS

Education and/or Experience:

· Bachelor's degree in Social Science related field preferred and two (2) years’ experience working in the Social Services field, or

· A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and equivalent experience working in the Social Services field.

Excellent communication, time management and project management skills. One (1) year supervisory experience preferred.

_Language Skills:
_Employee must have strong interpersonal skills, effective oral and written communication skills, and the ability to prepare clear, concise reports. They must possess working knowledge of computer software that includes databases, spreadsheets, and word processing for tracking information and preparing reports, letters, and forms. The ability to effectively present information, and respond to questions from clients, families, agency, county, and state representatives is also necessary.

Certificates, Licenses, Registrations:
Valid PA driver's license, Proof of valid Auto Insurance, ACT 33 (Child Abuse), ACT 34 (Criminal Record) and FBI Clearance. Employee must meet the requirement for becoming a Certified Community Health Worker established by the Pa Certification Board (PCB) within 1 year of being hired. There will be monthly screenings of PA Medicheck List, LEIE and EPLS.

Physical/Mental Demands:
While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly uses a computer, phone, fax, copier, and other essential office equipment to carry out daily work assignments.

Work Environment:
The Community Health Supervisor/Worker will work primarily in the community and will offer services wherever the client location may be; in the client's home, community center, and place of worship or treatment center. Community Health Supervisor/Worker will conduct screenings, provide resources, problem resolution and informal support network building for individuals served where and when it is needed. The Community Health Supervisor/Worker will be required to drive throughout Westmoreland County and its surrounding counties. Exposure to outside weather condition will exist. The noise level in the work environment is usually moderate.
