Overview:
LifeLong Medical Care is looking for a COVID Street Outreach Community Health Worker (CHW) in Berkeley. The Community Health Worker – COVID Street Outreach (CHW) is a member of the outreach team at LifeLong Medical Care. This position will work with schools, community organizations, neighborhood groups and others in Berkeley to ensure that everyone is educated about COVID-19 resources including vaccination and testing. The CHW will participate in door-to-door outreach and community events, in person and/or virtual.

This is a grant funded, full time, benefit eligible position. Must be bilingual English/Spanish.

Benefits

COVID-19 Vaccine Policy
In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Unless a reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.
Responsibilities:
  • Engage and educate residents on the streets, in schools, at their homes (door-to door canvassing) and other locations. Creating out-of-the-box ways of reaching individuals in order to share information about COVID-19 resources including testing and vaccination pop-ups and events.
  • Provide respectful engagement and education to parents of children 5 years and older and adults with the purpose of increasing testing and vaccination rates for COVID-19, specifically for the unvaccinated population.
  • Provide targeted outreach, education, and engagement linking the target population to insurance programs, wellness services and on-going primary care with LifeLong.
  • Collaborate and coordinate engagement activities and events with appropriate community agencies, local businesses and neighborhood groups.
  • Develop and maintain excellent working relationships with community partners and work together to identify opportunities to provide information about testing and vaccinations for the target population—African American and Latino residents of Berkeley.
  • Complete all appropriate data collection, paperwork, and tracking of activities in a timely manner.
Qualifications:
  • Commitment to the provision of services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
  • Basic computer skills, including Microsoft Word, Excel, Zoom and email.
  • Ability to effectively present information to others, including other employees, community partners and vendors.
  • Ability to work with individuals and organizations at the local level to build relationships.
  • Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
  • Work in a team-oriented environment with a number of professionals with different work styles and support needs.
  • Excellent interpersonal, verbal, and written skills.
  • Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
  • Be creative and mature with a “can do”, proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement.
  • Must be bilingual English/Spanish.
  • Must have access to transportation.
