Sparkles Home Health Care is seeking a reliable and consistent parttime ILS/IHS to work with clients in the comfort of their home and community. You will help individuals become integrated and engaged in the community while fostering the development of independent living skills. This position provides direct supervision to clients in their own homes or a community setting.
Individualized Home Supports uses a person-centered approach to support what is important to and for a person. The service combines both training and supports into a single, flexible service. Individualized Home Support is designed to provide in-home assistance to people who live independently in their own home. Individuals who are receiving support by Sparkles IHS program receive supports based on their level of need.
Individuals may need support ranging from 10-20 hours per week.
Duties include:
1. You will be providing direct one on one supervision, cueing, maintenance, guidance, instruction, incidental assistance with activities of daily living or assistance with coordination of community living activities to help the person live independently.
2. Help client with organization skills and other goals their want to achieve.
3. Promote client growth through achieving goals identified in their support plans including positive behavior support, vocational, communication, community integration, and other goals as specified.
4. Taking client to community event and errands.
Accountabilities
- Maintain an up-to-date and comprehensive knowledge on all related policies and procedures, rules and regulations for related to Maltreatment of Minors Act, Vulnerable
- Adult Act, Data Privacy, Documentation, Individual Rights, Bloodborne Pathogens and, Fraud Prevention.
- Manage and maintain all required documentation. Prepare weekly support plan.
- Responding to and reporting incidents as required for HCBS. Following safety practices established by SPARKLES.
* Staff must be COVID-19 vaccinated AND willing to provide proof of vaccination
Requirements
- 18 years or older.
- Must have empathy and patience to support clients
- Must be able to pass Department of Human Services background check
- Have a reliable source of transportation to and from the client's home.
- Must be familiar with 245D paperwork required by the State of Minnesota
Valid driver's license, acceptable driving record, and proof of auto insurance required
- Excellent verbal and written communication skills.
- Ability to react quickly, under high pressure with minimal supervision.
- Must be able to document client progress reports and meeting notes.
- Have A Minnesota Driver’s License, Full Coverage Insurance, and Reliable Vehicle.
- Be willing to transport clients in your vehicle
Pay: $ 17.00 - $20.00 per hour

- Day, Afternoon and Evening to fit your schedule

Experience:
- Caregiving: 1 year (Preferred)
- Driver's License Required
- CPR/Basic First Aid
