Job description
The Grand Rehabilitation and Nursing Center at Pawling is seeking an RN Admissions Coordinator for Evening Shift.
Flexible schedule || 3p-11p are core hours
Admissions Nurse Duties Include:
- Initiate admission assessments on new residents and hospital returns; initiates initial care plan.
- Assist with the direction of the Nursing Department as directed by the DON.
- Assess PRI's & coordinate components of the admission process with the interdisciplinary team.
- Audit records to ascertain compliance with facility standards.
- May be assigned to supervise, direct and/or provide Resident care as necessary.
Necessary Skills/Qualifications:
- Must have active & unrestricted NYS RN License.
- Excellent clinical assessment skills.
- Ability to make independent decisions.
- Strong leadership and supervisory skills.
- Computer savvy; proficient in MS Office (Excel, Word, Outlook).
- Long-Term Care Experience Preferred.
- Excellent communication and interpersonal skills.
