Job Summary
Chloeta is seeking a Family Nurse Practitioner or Physician's Assistant (FNP/PA) to provide outpatient and telehealth care at a medical facility in Chinle, AZ. Chinle provides medical care for approximately 35,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. In addition, most of our elderly patients speak only Navajo, and live according to traditional Navajo cultural practices.
Tour of Duty: 8:00 am to 5:00 pm - Monday through Friday. Overtime can occur if patient load requires. This position offers a short-term contract with long term options to extend one to three years.

Duties/Responsibilities
- Provide outpatient care in a compassionate manner to enhance the lives of the community.
- Conduct outpatient clinics; make observations appropriate to the outpatients underlying disease and treatment plan including current symptoms.
- Review the patient's medications, history, diet, therapy course, and adherence to management plan and patient observations. Modify regimen as required. Instruct and counsel patients as needed; perform diagnostic tests and examinations deemed necessary to obtain all possible information related to each case.
- Make preliminary diagnosis, direct, prescribe or provide treatment or arrange for specialized care by patient referral as needed.
- Provide emergency care as required; provide definitive management in all medical categories for cases that do not require referral.
- Give total patient care including prevention, health maintenance, early diagnosis, treatment, and follow-up services to patients under incumbent's care.
- Provide telemedicine services for patients with mild respiratory illness in the respiratory clinic. These services will be rendered on the premises but the NP/PA may not be physically present with the patient. From time to time, based on the patient’s clinical condition, the NP/PA may do a brief hands-on assessment in person with these patients as needed.
- Provide telephone visits for patients in need of chronic care management for whom it would be high risk to travel to the hospital for care.
- Refer patients to appropriate contracted medical or other facilities providing full clinical information for care and diagnostics procedures that cannot be adequately provided at the hospital. Coordinate and integrate information obtained into on-going health care program at the hospital and assure the provision of timely follow-up care as provided.
- Advise the medical officers in other services concerning these operations. Be in complete charge of guiding and directing the assisting medical officer and nurse.
- Provide consultation on difficult cases to practitioners in the same or other specialties. Work closely with allied health professionals in a collaborative effort.
- Utilize the EHR system to obtain, dictate, electronically enter, and assure preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Clinical data reports shall be completed in a timely manner and shall be maintained in compliance with Joint Commission standards.
- Be familiar with the cultural, social, political, geographic, demographic, economic, and epidemiological characteristics of the patients and the communities served. Be receptive and respectful of the culture. Cultural awareness and cultural diversity include language barriers, rural Native lifestyles, and native ceremonies. Be aware of the basic traditional medicines and ceremonies that are available to patients, from the womb to geriatrics. This unit has traditional providers on staff providing traditional diagnostic care for all patients who seek it.

Education Qualifications
- Required: The FNP or PA must have completed an accredited program in the U.S.A..
- Required: A minimum of six (6) months experience in the specialty required by the contract. There must be no lapse of services of more than six (6) consecutive months for at least six (6) months prior to beginning tour of duty.
- Required: Be board certified with the American Nurses Credentialing Center or the American Association of Nurse Practitioners or the National Commission on Certification of Physician Assistants that is appropriate to specialty.
- Required: Must possess a current, valid, unrestricted, applicable medical license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract.
- Required: Must maintain a valid, current, full, and unrestricted DEA license with schedule types II through V and maintain certification throughout the term of the contract. License verification must be provided at the time of application.
- Required: Shall maintain the following valid and current certifications:
- Cardiopulmonary Resuscitation (CPR),
- Basic Life Support (BLS),
- Advanced Cardiac Life Support (ACLS),
- Advanced Trauma Life Support (ATLS), and
- Pediatric Advanced Life Support (PALS).
- Required: Provide three current letters of reference from other health care providers familiar with the contractor's clinical practice skills within the same discipline. One letter must be from the Chief of Staff/Medical Director or Immediate Supervisor at the hospital in which the contractor either holds or last held staff privileges. If residency program completions was recent, one of the letters shall be from the program director or service chief.

Physical Requirements
- Must be able to withstand physical and emotional stress.
- Requires long hours of sitting, standing and moving.
- Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
- At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
- At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:
This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.
Drug and Alcohol Testing:
To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.
Fair Credit Reporting Act (FCRA):
Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
Emergency Response:
Every Chloeta employee has regular and recurring emergency management responsibilities, though not every position requires routine deployment to disaster sites. All positions are subject to recall around the clock for emergency management operations, which may require irregular work hours, work at locations other than the official Duty Station, and may include duties other than those specified in the employee's official Position Description (PD). Travel requirements in support of emergency operations may be extensive in nature (weeks to months), with little advance notice, and may require employees to relocate to emergency sites with physically austere and operationally challenging conditions.
