We are looking for a compassionate Home Health Registered Nurse/Clinical Manager to fill a new position within our setting. Applicants will be expected to learn and be proficient with current EMR, able to complete HH nursing needs, complete Admission, Resumption of Care, and DC oasis in a timely manner, participate in on-call schedule, and operate with minimal supervision. As the Clinical Manager, it is their responsibility to ensure that patient care is coordinated and managed appropriately as well as complete patient care and Home Health admissions. This position is also responsible for patient care a portion of the day or week.

Clinical Manager Job Functions include, but are not limited to:

· Receives case referrals. Reviews available patient information related to case, including home visits, to determine home care needs.

· Conferences with MEDICAL DIRECTOR regarding any questions about an individual’s eligibility for services.

· Reporting to the Director of Home Health and Administrator, support the clinical team workflows and provides direct supervision to Certified Nursing Assistants, if available.

· Involved in day-to-day patient activities 50% of the time at minimum, including phone and in-person patient triage and patient education

· Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.

· Reviews patient’s medical diagnosis, prognosis, medications, procedures and clinical course.

· Follows up and communicates to home health staff regarding missed/additional visits, patient orders, and clinical documentation needs.

· Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.

· Attends case conference meetings with personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.

· Conducts daily and weekly record reviews and communicates findings and recommendations to Director of Home Health and personnel.

· Supports scheduling for all home health care staff, including nursing, therapy and ancillary staff.

· Assists the Administrator and other supervisory personnel in the planning, implementation and evaluation of in-service and continuing education programs.

· Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, QAPI goals and objectives, both short and long range.

· Complies with accepted professional standards and principles.

· Promotes customer service orientation to all personnel

· Oversees clinical quality and patient safety through training and education of the back office personnel and administrative staff. Works closely with the Market clinical and operations leaders to ensure day-to-day clinical efficiencies in the center.

· Participates actively in quality assessment performance improvement program.

· Performs other duties and activities as delegated by the Regional Director of Operations or President of Long Term Care.

Minimum Qualifications:

· Graduate of a Registered Nursing accredited school of nursing with a BSN degree is preferred.

· Previous experience in hospice, home care setting and with two years management or supervisory experience. Proven ability to work within an interdisciplinary setting.

· Registered nurse with current licensure to practice professional nursing in the state.

· Complies with accepted professional standards and practice.

Job Type: Full-time

Pay: $30.00 - $36.00 per hour
