Home Health Nurse Case Manager (RN)

*** MUST HAVE 1 YEAR OF HOME HEALTH EXPERIENCE TO QUALIFY ***

Coverage areas: Shaker Heights, Ohio area

Travel allowance: $100 per week ($5200 yearly)

On Call: One week per month. Clinical Calls only. Scheduler the takes first calls. $150.00 allowance when on call.

Summary of Position Responsibility:

The RN Case Manager assesses patients and their needs. They manage the patient’s case and make appropriate suggestions regarding pain and symptom management in order to ensure comfort and quality of life. The RN Case Manager educates and supports patients and families regarding expectations during the end-of-life process and is responsible for the overall coordination of patient’s care.

Essential Duties and Responsibilities:

  • Provides patient care visits for admission, assessment and pain and symptom management and support during the dying process

  • Educates current and prospective patients and families regarding hospice and the end of life

  • Documents patient visits and phone calls in an accurate and timely manner

  • Reviews and updates care plans, medication and equipment orders and completes orders of new medication and supplies for patients

  • Monitors visits and calls of certified nursing assistants

  • Updates the patient’s physician and/or medical director regarding any changes in status

  • Attends and participates in interdisciplinary team and RN meetings regarding patients and families and collaborates with the interdisciplinary team appropriately

  • Assesses family and caregiver ability to provide safe care, carry out care plan and educate on the proper functioning of all equipment

  • Enhances self knowledge regarding new medical and hospice information.

  • Assists with on-call duties

  • Other duties may be assigned or required for the performance of this position

Education & Formal Training Required:

  • Associate Degree in Nursing

  • Bachelor of Science in Nursing

Certifications, Licenses & Registrations Required:

  • Licensed Registered Nurse in the state of Ohio

  • Valid Driver’s License

  • Valid Auto Insurance

Job Experience Required:

  • Minimum of 1 year clinical experience, preferably in an acute care or Home Health or Hospice setting

Specialized Competencies (Skills, Abilities, and Behaviors) Required:

  • Ability to demonstrate sound clinical judgement with minimal supervision

  • Ability to function independently, organize and prioritize

  • Ability to work under pressure, multi-task and maintain a calming, professional presence in a fast-paced environment with frequent interruptions

  • Flexibility to embrace change and effectively shift priorities

  • Communication skills, verbal and written

  • Organizational skills

  • Computer skills (Word, Excel, patient database, e-mail), voicemail

  • Knowledge of federal and state healthcare regulations, preferred

Physical/Mental/Visual Job Requirements:

  • Frequent standing, walking, bending, crouching, and reaching with hands and arms

  • Occasional sitting and working at the computer including dexterity in hands for typing and vision to allow adjustment of focus

  • Occasional assistance in administering medications that is ordinarily self-administered

  • May be required to lift up to 25-50 pounds

Working Conditions:

  • Possible exposure to communicable diseases

  • Frequent driving to complete patient visits, including traveling in bad weather
