Title: Home Health Physical Therapy Assistant (PTA)

Reports to: Clinical Manager

FTE: Full-Time

Salary: Up to $33/hourly

Duties & Responsibilities:

• Provides rehabilitative and skilled PT services to patients in their place of residence under the orders of an overseeing Physical Therapist

• Performs manual and functional therapy exercises and activities with the patient

• Report any changes in patient condition to the overseeing Physical Therapist and other members of patient’s multi-disciplinary team

• Assists in the supervision of CNA's

• Uses various tests to determine patient's individualized needs, and revises the plan of care as needed under direction of Physical Therapist

• Completes Point of Care Documentation

• Meet agency productivity expectations

Job requirements:

• Graduate of accredited Physical Therapy Assistant education program

• Current Oregon PTA license

• Current & valid OR Driver's license

• Current & valid car insurance

• CPR certified

• Dependable car

• Ability to function as a member of the Interdisciplinary team and communicating weekly

• Excellent critical thinking skills

• Previous Home health experience preferred
