Integrated Health Resource Specialist is responsible for the coordination or liaison to services for individuals with complicated behavioral health needs. The target client population would consist of identifying patients with chronic and persistent mental health diagnoses that need referrals to the Columbia CCBHC provider for CPRC and psychiatric services. The IHRS will also facilitate internal behavioral health referrals for the central region. This position will collaborate with the CCBHO Care Coordinator for referrals on a regular basis and will pursue feedback and tracking for referred patients.

Responsibilities include:

• Identify, refer, and follow up for patients with a chronic and persistent mental health diagnosis that need a referral to the Columbia CCBHO provider for CPRC and psychiatric services
• Identify, refer, and follow up to coordinate internal CHN behavioral health referrals for services for individuals with complex behavioral health needs
• Provide engagement and reengagement to assist identified consumers with service participation
• Collaborate with clinical staff to assure engagement, coordination, and successful referral tracking of identified consumers needing behavioral health treatment, including routine follow up with referral CCBHO Care Coordinator, and other patients requiring internal referrals. Provide or coordinate training and consultation with medical and behavioral health providers on for treatment options
• Provide data collection, tracking, and other duties as assigned
  
  
  
• High School/GED required
• Preference given to individuals with a bachelor's degree in a human service-related field working toward a master's degree and behavioral health licensure
  
  At least five (5) years' experience required if bachelor's degree in a human service-related field is not obtained
