Job Title: LVN Care Coordinator- TX (Midland & Odessa)
Location: Midland & Odessa, TX
Duration: 3 Months (Extendable)
Shifts: 08:00 AM - 5:00 PM
Requirement:
- Bachelor’s degree in Social Work, Health Services, Behavioral Science or related field, or equivalent experience. LVN all accepted.
- 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
Description:
- SC will have 3 days of visits and 2 days of documentation weekly
- SC will have a caseload and work assigned tasks
- SC will complete H2060 assessment
- SC will fill put required forms for each assessment
- SC will make follow up calls
Job Types: Full-time, Contract
Pay: $22.00 - $28.00 per hour
