The manager of Case Management coordinates activities within case management that provide for cost-effective and appropriate use of healthcare resources. The manager will also work with other departments to assure that the hospital is fairly reimbursed for medically necessary and reasonable services that are provided to patients.

Case Management/Care Coordination: Coordinates activities related to case management. Oversees schedules, hiring, evaluation, and competency testing of Care Coordinators. Utilization Review: Coordinates the various tools available for managing resource utilization within various care settings. Assists with the coordination of activities related to utilization review. Provides education and leadership to the Care Coordinators in order

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

  • Bachelors Degree in Nursing required

  • Current, valid Michigan Nursing license required

  • 3-5 years current experience in the areas of case management, discharge planning, utilization review, or a related field.

  • Appropriate certifications preferred.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Assists with the development of annual goals for the case management program. Verbalizes and models the behaviors necessary to attain these goals to the case management staff.

  • Ensures that staffing is sufficient and appropriate to meet both operational and budgetary goals. Adjusts staffing levels as appropriate for patient volume and acuity. Maintains payroll records.

  • Prepares and participates in reporting as needed and requested. Oversees hospital LOS and CMI.

  • Fills in as necessary in emergencies in all departmental positions. Maintains the in-depth practical level of knowledge necessary to do so.

  • Works with case management staff to develop an annual “Impact Plan” as a way to build employee satisfaction and engagement.

  • Actively supports patient satisfaction initiatives. Shares appropriate satisfaction data with case management staff and develops plans to impact related patient satisfaction scores.

  • Performs all duties and responsibilities according to the organization’s service excellence standards.

  • Provides education, support and expertise necessary for case management staff to meet their professional goals.

  • Maintains personal professional growth and development necessary to keep abreast of current trends and techniques

  • case management, utilization review and clinical documentation. Maintains current knowledge regarding federal and state regulations, legal updates, and third party payer requirements.e

  • Develops and provides appropriate and ongoing competency evaluations to case management staff. Ensures that competency evaluations are timely and contain appropriate age-specific skills.

  • Conducts an annual educational needs assessment and determines educational priorities for the case management staff. Provides or provides for needed education.

  • Supports, leads, or participates in a variety of committees, teams or groups as needed by current hospital and department goals.

  • Actively participates in any six sigma or other improvement projects related to the area of case management. Attends all necessary meetings and contributes an in depth knowledge of departmental processes.

  • Participates in other hospital committees as requested.

  • Provides appropriate educational updates as necessary to hospital and medical staff. Interacts with the medical staff as an expert in clinical documentation.

  • Interviews, orients, trains, directs, evaluates and recommends the hiring, discipline and/or discharge of casemanagement personnel.

  • Prepares objective, timely performance evaluations. Utilizes goal-setting techniques. Assists in developingappropriate measurable standards of performance.

  • Ensures staff compliance with hospital and regulatory requirements.

  • Performs a variety of other management tasks to ensure optimal delivery of services.

  • Reviews and develops appropriate departmental policies and procedures. Ensures that employees are informed of newdepartmental and hospital policies and procedures.

  • Works closely with the Physician Advisor to facilitate effective processes that will ensure appropriate use of hospital

  • resources.

  • Assists in the development of an appropriate departmental budget.\

  • Facilitates a denials management process.
