Description/Job Summary

The RN Care Manager (Clinical Nurse) for HIV/Hepatitis C and PrEP reports to Clinical Lead NP for HIV/HCV. The Clinical Nurse is responsible for but is not limited to: patient triage, patient assessment, patient education and counseling. The Clinical Nurse ensures timely and productive patient flow within Primary Care and connects patients to services as needed.

Responsibilities/Duties

  • Assure that all operating procedures and regulations are followed.
  • Provide primary care services to patients of all ages, including phone/walk-in triage, patient education, counseling, family planning activities, coordination of appointments and referrals, assistance in obtaining needed services and supplies.
  • Work collaboratively with the Primary Care Nurse Practitioners and the Primary Care Department and teams to provide comprehensive primary care for all patients.
  • Provide direct patient care activities within the scope of the Registered Nurse including blood pressure checks and hypertension follow-up visits, diabetes follow-up and teaching, other chronic care management visits, emergency department and hospital admission follow ups, medication injections, patient-centered education, and other tasks as delegated and assigned by the Primary Care Nurse Practitioners.
  • Care Manage a designated list of high-risk patients referred by insurance lists, i2i lists, care coordinators, or by a nurse practitioner. These are patients with more than one chronic condition (HTN, COPD, Asthma, CHF, DM1/2, HIV) at risk for hospitalization, and in need of assistance in overcoming psychosocial barriers to care and navigating referrals and follow-up care.
  • Work collaboratively with the HIV/HCV Clinical Lead and the HIV care team to provide comprehensive HIV care for all HIV+ patientsFacilitate all new HIV patient appointments in conjunction with the Call Center, the Annex West PSR, and the Action Wellness case managers
  • Coordinate response for newly diagnosed HIV patients across the network per the iART Rapid Start Protocol
  • Facilitate management of the HIV patient population within the Primary Care setting Participate in follow-up and reminder calls/letters for all HIV+ patients, ensuring regular follow up
  • Track compliance with performance measures related to HIV care for all HIV+ patients Provide care management to all HIV+ patients as deemed appropriate (ER use/hospitalizations)
  • Provide services to HIV+ patients, in cooperation with the patient RN for each site, including phone/walk-in triage, education, counseling, family planning activities, appointment scheduling and referrals, assistance in obtaining needed services and supplies
  • Assist in referring patients to other needed services including Behavioral Health, Social Work and Outreach and Enrollment
  • Provide comprehensive, individualized health education and adherence counseling to HIV+ patients
  • Manage patients on Cabenuva (HIV treatment) or Apertude (HIV prevention) by obtaining prior authorization as needed, scheduling appointments for injections, and providing injections as needed
  • Work with the HIV Quality team, including but not limited to the Quality Manager, the Program Manager, the Clinical Lead for HIV, to improve HIV services and access to PrEP, and to provide funders with all necessary data and reports, including data entry using CareWare
  • Identify patients diagnosed with Hepatitis C either through provider referral or monthly reports; assist in scheduling HCV patients with providers, obtaining labs, obtaining prior authorization for medications, facilitating the delivery of medications, and tracking patients through the treatment process until complete

Required Experience

  • 5-7 years professional nursing experience required
  • Outpatient and/or birthing center experience
  • Previous supervisory experience a plus
  • Previous experience working with an Electronic Health Records database.
  • Ability to communicate professionally in both verbal and written forms.
  • Ability to work independently and manage multiple expectations.
  • Proficient computer skills.

Required Education

  • Bachelor of Nursing Degree from an accredited institution

Required Qualifications

  • Bachelor of Nursing Degree from an accredited institution required.
  • Current unrestricted Registered Nurse with appropriate State License; Commonwealth of Pennsylvania required.
  • At least 5 years’ experience as a RN required.
  • Experience in an outpatient practice setting preferred; experience working within OB for pre-natal nurse.
  • Previous experience working with an Electronic Health Records database.
  • Knowledge of primary health care and the social determinants of health
  • Knowledge of concepts of health promotion, disease prevention, behavior change counseling, program planning, individual and group counseling.
  • Nursing care knowledge related to primary care/community health/chronic disease management
  • Excellent organizational skills to manage numerous practice activities.
  • Demonstrates initiative, leadership and a high level of nursing skills.
  • Unconditional ability to maintain HIPAA regulations.
  • Leadership skills that include the ability and desire to champion teambuilding within the
