Job Type/Status: Part Time, 10 hours per week
Position Summary
Center For Family Services is seeking a Part Time Case Manager. The Case Manager provides direct case management services and may perform specialized activities and functions. The Case Manager will be responsible for oversight and monitoring program implementation for the identified grant provider. The role of the Case Manager will be to provide support, guidance, and coordination to the Peer Support Teams to ensure program outcomes are being met, clients are accessing treatment/services and meeting identified goals
Duties and Responsibilities

• Provides direct case management services
• May perform specialized activities and functions specific to program
• Completes documentation and other reporting requirements
• Meets regularly with supervisor, and participates in other continuing professional development opportunities
• Attend weekly Peer Support case conferences convened by the Grant Project Manager as well as the bi-monthly Camden County Reentry Committee meetings.
• Contributes on agency committees and special tasks and promotes a sense of agency with co-workers and staff, and communicates in ways that support agency mission and values

Qualifications

• Bachelors Degree in Human Services with at least 3 years experience in substance abuse, Masters degree preferred with two years experience in substance abuse.
• Shall possess knowledge in formal/informal community systems and resources
• Engage in continuing professional and personal development relative to recovery support services
• Must have understanding of Medication Assisted Treatment and recovery support services.
• Must have knowledge or experience with Peer Recovery Support Services.
• Computer skills, including spreadsheet and database application or maintaining data and generating reports.
• Shall possess knowledge in formal/informal community systems and resources.
