Position Overview: We are seeking a highly skilled and compassionate freelance Physical Therapist to provide exceptional therapy services to an elderly couple in their aging in place home.
As a freelance Physical Therapist, you will play a vital role in promoting the couple's overall health and well-being by implementing personalized treatment plans and assisting them in recovering and maintaining their mobility, strength, and independence. Responsibilities:
Assessment and Evaluation: Conduct assessments to evaluate the couple's physical capabilities, mobility challenges, and specific therapy needs. Develop a thorough understanding of their medical histories, conditions, and limitations that affect their therapy. Review recommendations from Hamilton Home Health, etc.
Treatment Planning: Design personalized treatment plans based on the couple's individual needs, goals, and preferences. Collaborate with the couple, their healthcare team, and family members to establish realistic therapy objectives.
Therapy Sessions:Conduct regular therapy sessions in the couple's aging in place home, ensuring a safe and comfortable environment. Utilize a variety of physical therapy techniques, exercises, and modalities to improve strength, balance, flexibility, and mobility.
Teach and supervise the couple and their caregivers in performing prescribed exercises, ensuring proper form and technique. Monitor their progress, make adjustments to the treatment plan as needed, and document their outcomes.
Education and Counseling: Educate the couple on preventive measures, self-care techniques, and strategies to enhance their functional abilities and independence. Provide guidance and support to family members, caregivers, and the couple on managing daily activities and creating a safe home environment.
Communication and Collaboration:Maintain regular communication with the couple, their healthcare team, and any involved specialists or therapists. Document and share progress reports, treatment updates, and any concerns or recommendations to ensure coordinated care.
Requirements:
- Valid Physical Therapist certification.
- Proven experience working as a Physical Therapist, preferably with older adults and/or in a home care setting.
- In-depth knowledge of geriatric physical therapy, including age-related conditions and rehabilitation techniques.
- Ability to assess, plan, and implement effective therapy interventions.
- Strong communication and interpersonal skills to establish rapport and build trust with the elderly couple and their support network.
- Empathy, patience, and a genuine passion for working with older adults.
- Self-motivated and capable of working independently, managing time effectively, and prioritizing tasks.
- Reliable transportation to travel to the couple's aging in place home.
Both members of this couple have recently had injuries or undergone treatments and surgeries that have limited their mobility and strength. They are mentally and emotionally independent and want to gain as much strength and mobility as is possible at this time.
