Responsibilities:
- Providing limited term skilled nursing services to individuals returning home from higher level care facilities
- Assessing, generating, and evaluating patient care plans and patient progress
- Liaising with other members of each patients multi-disciplinary care team
- Recording patient progress in HCHB via tablet
- Attending regular staff meetings
Requirements:
- Current NJ RN license
- Reliable transportation, drivers license, and insurance
- Strong interpersonal and customer service skills
- Ability to demonstrate knowledge of electronic documentation
- Additional training certificates a plus
- Preference for nurses with prior Home Health experience including: Psych, Vent, Peds, Admissions

Weekly schedule:
- Monday to Friday
- Rotating weekends
Experience:
- Nursing: 1 year (Preferred)
License/Certification:
- RN License (Preferred)
