JOB SUMMARY
At Houston Methodist, the Patient Safety Specialist I takes an active role in the implementation of the Quality and Patient Safety Plan and the daily operations of the Patient Safety Program. The Specialist works with others within the facility to foster a culture of patient safety and continuous improvement and coordinates systematic implementation of effective practices designed to reduce error, facilitate the creation of reliable systems and processes, identify improvement opportunities, and mitigate risk to improve patient outcomes. The Specialist serves the organization as a leader of patient safety improvements by demonstrating expert knowledge and ability to execute change as well as mentoring other staff, physicians and peers in the best practices of patient safety. The Specialist understands and contributes to the strategic plan for patient safety.
PRIMARY JOB RESPONSIBILITIES

PEOPLE - 25%
Leads facility partners to bring expert assessment and problem solving skills to ensure reliable, safe systems for all patients, physicians and staff. (EF)- Develops and maintains positive working relationships with leadership, physicians, colleagues and peers and works collaboratively to achieve desired outcomes. (EF)
- Collaborates and effectively communicates and drives culture of safety initiatives and partners with leadership and clinicians to implement improvements. Increases Patient Safety awareness and practice among clinicians and staff through mentoring and education. (EF)
- Manages project and process improvement expectations to stakeholders, sponsors and others advising them on project progress, potential issues, obstacles, conflicts or challenges. (EF)
SERVICE - 20%
- Disseminates meaningful and useful reports and provides recommendations to leadership, quality teams and point of care individuals. (EF)
- Supports leadership with the development and implementation of patient safety process changes. Makes recommendations for unit-based process change activities. Evaluates the effectiveness of process change initiatives. (EF)
QUALITY/SAFETY - 40%
- Facilitates the root cause analysis [RCA], failure mode and effects analysis [FMEA], patient safety review [PSR] and other review methodologies. Performs the review of medical information, interviews staff, debriefs all individuals as indicated and ensures timely follow-up for identified action items. (EF)
- Facilitates and leads process improvement project teams and initiatives. Participates in unit-based and department process change activities. Evaluates the effectiveness and sustainability of process change initiatives and makes changes as necessary to achieve goals. (EF)
- Sets standards for the measurement, analysis, and reporting of adverse trends for review by appropriate stakeholders and assists in development of action plans for resolution. Monitors action plans and facilitates revisions and evaluates sustainability of change. (EF)
- Acts in a leadership role around patient safety functions, including assessment, implementation, proactive planning of error reduction strategies and patient safety communication. (EF)
- Focuses on implementing and reinforcing the principles that support a high-reliability organization. Contributes to the continued improvement of patient safety practices, employs evidence based practice and researches high reliability practices through National Patient Safety Organizations [AHRQ, NPSF, IHI, NQF].
- Tracks, analyzes, and uses data for trending and develops appropriate action plans and risk modification strategies in collaboration with clinicians and leadership. Advises on data and when it is no longer necessary to actively drive change.
FINANCE - 5%
- Utilizes efficient and cost-effective work practices with department resources and supplies; provides recommendations to reduce expenses. (EF)
GROWTH/INNOVATION - 10%
- Communicates innovative and best practices to hospital leadership and clinicians. Identifies opportunities to align policy and procedure with regulatory/accreditation requirements. (EF)
- Translates patient safety analyses into meaningful information to provide direction and drive action. (EF)

EDUCATION REQUIREMENTS
Bachelor of Science in Nursing
EXPERIENCE REQUIREMENTS
- Four years of experience in direct patient care activities in a hospital setting required
- Three years of experience in hospital patient safety/performance improvement/risk management preferred
CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED
Texas RN license or temporary TX RN license, should obtain permanent license within 90 days. Compact license acceptable according to current Board of Nursing requirements- Certified Professional in Healthcare Quality (CPHQ), Certified Patient Safety Officer (CPSO), or Certified Professional in Patient Safety (CPPS) preferred
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations- Sufficient proficiency in speaking, reading, and writing the English language necessary to perform the essential functions of this job, especially with regard to activities impacting patient or employee safety or security
- Ability to effectively communicate with patients, physicians, family members and co-workers in a manner consistent with a customer service focus and application of positive language principles
- Ability to identify and define problems, collect data, establish facts and draw valid conclusions
- Ability to enter and abstract data using personal computer, as well as the ability to utilize data to facilitate the improvement and change in processes (Access, Excel, Word)
- Ability to facilitate patient safety initiatives, effectively present data and promote a collaborative approach toward goal achievement; skilled in teaching and consultation
- Demonstrated knowledge and application of process improvement tools and techniques (statistical process control tools and team tools)
- Demonstrated knowledge of regulatory and accrediting standards as they apply to patient safety
- Skill in developing and maintaining interpersonal relationships with a wide variety of healthcare professionals and hospital leadership to achieve a common goal
- Ability to work independently and within teams to complete tasks and achieve goals
- Ability to mentor others
- Skill in data collection/aggregation, data analysis and data reporting
- Clear understanding of the timeliness of organizational reporting regulatory events to regulatory bodies
- Excellent written, oral and presentation skills to ensure clear communication with stakeholders
