Summary

The Social Services Liaison is primarily responsible for assisting Millennium employees in understanding what type of community resources are available to Millennium's patients, coordinating the community resources as appropriate, understanding what type of resources are available for patients who need assistance with bridging the gap of pharmaceutical copays, and coordinating with the appropriate resources to assist the patient. S/he must exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish the goals and objectives.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Research, and otherwise gather information regarding community resources, including pharmaceutical resources for patients needing assistance in paying for copayments;
  • Organize the available information in such a manner or as directed, to share the resources with other Millennium employees to assist the patient as appropriate;
  • Support Millennium's employees in the coordination of the community resources to effective support the patient's care plan; and
  • Meet with community resources within the designated area to build relationships to assist patients in obtaining those resources;

Education and/or Experience - Bachelor's degree from four-year college or university; preference for a bachelors or masters in social work; or one to two years related experience and/or training; or equivalent combination of education and experience.

Language Skills - Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints.

Mathematical Skills - Ability to apply basic math, as necessary.

Reasoning Ability - Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills - To perform this job successfully, an individual should have knowledge of Athena Database software; Microsoft Spreadsheet software and Microsoft Word Processing software; including Excel.
