Northland PACE Senior Care Services is looking for Social Workers (LBSWs or MSWs) that love to work with the elderly!
PACE is a Medicare program and Medicaid state option that gives community-based care and services to people 55 or older who otherwise would need a nursing home level of care. The heart of the program is a team of professionals called Inter-Disciplinary Team (IDT) that coordinates all aspects of healthcare either at our Day Center Clinic or in our participant's homes to enhance their quality of life.
As a member of the IDT, the Social Worker promotes and maintains the mental and social health of enrolled participants through assessment, treatment, teaching and counseling. They provide basic casework and consultation for our participants by facilitating communication between participants, their family and the Care Team. They are responsible for implementing a social work care plan and coordination of social work with other services.

Employment Specifications:
- Education/Experience: Master’s Degree from a school of Social Work accredited by the Council on Social Work Education.
- Licensure, registry or certification required: Must possess current, valid SW licensure in the state of ND. Must maintain current licensure/certification within scope of practice.
