Qualifications: Bachelor's Degree in Human Services related field
STUDENT LEVEL RESPONSIBILITIES
- Gain an awareness of all at-risk students in building (read case summaries and communicate updates, participate in STAT, participate in SEEK)
- Help coordinate K-4 at-risk services with building administrators and nurse
- Provide Interventions by meeting individually and/or in small groups with students to address physical, social, behavioral, emotional, and academic needs (SAS sessions, RTI/MTSS Tier 2 and Tier 3, referrals for testing including social work reports)
BUILDING LEVEL RESPONSIBILITIES
- Consult with staff regarding behavioral/emotional interventions for student and class needs (RTI/MTSS Tier 1)
- Have an overall awareness of all behavior plans implemented in the building
- Consult with staff to help create, communicate, and implement non-special ed behavioral plans
- Monitor screenings used for ADHD, behavior, emotions, etc.
- Within the first 2 weeks of school, inform staff about student needs from last school year, such as behavior plans, SAS needs, and any other relevant history such as past interventions and case summaries
- Provide SAS classroom intervention programs
- Bullying – by Oct. 15th
- Sexual Abuse Awareness – by Dec. 15th
- Other topics as appropriate (relaxation strategies, coping, emotional/behavioral/social health)
- Lead the PLUS program through organizing student referrals and paperwork, pairing students, and communicating with partners throughout the semester
- Aid in organizing the Boomerang Backpack program through maintaining a list of participating students and distributing bags as needed
CORPORATION LEVEL RESPONSIBILITIES
- Consult with corporation staff as appropriate regarding individual student needs or information
- Provide information to staff at next school (including moves within the same school year)
- Participate in at-risk/SST meetings
- Communicate with nurses for Christmas families
- Coordinate building based McKinney Vento information and collaborate with Abby/Sherrie regarding McKinney Vento/homeless
- Communicate with corporation staff about PLUS program needs such as event planning
FAMILY LEVEL
- Consult with parents regarding their child’s physical, social, behavioral, or emotional needs
- Complete parent interviews and/or social background history
- Communicate with parents regarding social/emotional/behavioral ratings and collect these as needed
- Send SAS summaries to parents when sessions are completed, or at the end of the school year
- Make home visits to connect with parents and facilitate communication and interventions between home and school as requested by principal
- Help parents complete disability/Medicaid paperwork, including gathering all school records and information that is needed, as appropriate
- Help parents complete applications for Head Start Preschool services as appropriate
COMMUNITY LEVEL
- Consult with and gather information from medical, mental health, and other outside agencies
- Help facilitate referrals for services, between school staff, parents, and agency staff (including mental health, medical, DCS, etc.)
- Attend outside agency meetings for specific students when appropriate
- Monitor documentation for release of information, confidentiality, etc. for all non-special ed students (and SPED students as requested) and provide copies to staff as appropriate (nurse, teachers, principal, Lisa, Sherrie, Abby, etc.)
