Currently seeking a Transition Coordinator for our AHRC Middle/ High School located in Dyker Heights.
Position Requirements:
- NYS Certification in Pupil Personnel Services/Career and Technical Subjects/Special Education or related fields required.

Role Description:
- Full Time | Monday-Friday
- Hours: 8:00 A.M-4:00 P.M
- Initiate and coordinate efforts to establish community based job experiences and vocational opportunities.
- Develop and implement individualized educational/vocational/transition goals.
- Develop post-secondary linkages for transition services to students with disabilities to enable comprehensive planning for achieving independence, with a focus on vocational training, employment preparation, self-advocacy, integration in the workplace and community and/or adult services.
