Treatment Program Coordinator
The Treatment Program Coordinator holds accountability for the ongoing operations and successful coordination of treatment services through our SUD/MAT Programs. which is part of the Community Action Committee of Pike County. Under the direction of the Director, Health Center Operations, the Treatment Program Coordinator helps to operationalize the mission, vision, and values of the Community Action Committee of Pike County and Valley View Health Centers by ensuring efficient, effective, and patient-focused operations. This position serves as an advocate for VVHC’s treatment services in the community, which includes acting as a liaison to the local court systems, community partners, VVHC providers, case managers, therapists, patients, and our leadership team, in order to serve the best interest of our patients.
Utilizes the strategic plan and direction of CAC/VVHC to develop high-impact, patient-focused solutions around treatment services provided through our MAT/SUD programs.
Develops and nurtures positive working relationships with key stakeholders, including loal courts, recovery services, community partners and agencies, and probation officers.
Seeks innovative solutions to address patient needs.
Sources and explores additional funding opportunities and assists the Program Director in developing funding proposals and related reporting.
Advocates in our communities for expanded reach of testing and treatment services for Hepatitis C, as well as for Medication Assisted Treatment (MAT) for addiction.
Ensures compliance of MAT/SUD Program’s with legal, regulatory, and funding source requirements.
Thoughtfully and intentionally contributes to an organizational culture of excellence in all things.
Will provide CMHC services such as SUD, Case Management, and Peer Recovery Services as needed.
Collaborates with the other programs and departments to identify and address the needs of the targeted population
Researches, collects, and analyzes data regarding program performance and reports to Director, Health Center operations.
Assists with the implementation of new programs, services and initiatives as needed.
Responsible for the growth of MAT/SUD Programs within our service area.
Provides case management services to the targeted patient population
Works with team members to resolve conflict in a way promoting collaboration and respect.
Serves as part of the SUD/MAT Leadership Team and collaborates to promote program activities, service coordination, and continuous improvement measures.
Serves as a liaison in the courts to improve growth in related programs.
Serves as a representative of SUD/MAT Programs/VVHC on assigned committees and outside boards and associations as deemed appropriate.
Perform any and all other tasks as directed by Supervisor.
Minimum Requirements
High School Diploma
Minimum of 2 years of experience providing case management services.
Valid Driver’s license and must be insurable under the agency insurance
Preference given to individuals holding an associate degree in an appropriately related field of study.
Preference given to individuals with at least one year of experience in MAT/SUD setting.
