Admissions Nurse (RN)
Location: Atlantic County (Atlantic City, NJ)
Shift: Full-Time
Salary: $78,000-$88,000 depending on experience
Requirements:
- Possess a current license to practice as a registered nurse.
- Two years of work experience as a registered nurse preferably with experience in home health and/or hospice setting.
- Possess a valid drivers license, valid auto insurance, and reliable transportation for field use.
- Ability to provide skilled nursing services and complete documentation within the agencys productivity standards.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
- Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
- Ability to work as part of a comprehensive interdisciplinary team (IDT), and to represent the agency in the community.
- Must maintain a professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers, and all others supporting the mission and vision of the organization.
- Must be able to demonstrate optimism, enthusiasm, and willingness to work constructively with other team members and reflect concern for the well-being of others.
- Possess satisfactory references and pass criminal background, national sex offender, drug and motor vehicle checks.
Responsibilities:
- The Admissions Registered Nurse is responsible for assuring that our programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a patients home or in a health care facility. This position works with the intake team, referring physician and other referral sources to promote the hospice philosophy of care.
- The Admissions Registered Nurse provides skilled nursing care to patients in the facility and/or home setting as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, State licensure laws, accrediting body standards and agency policies and procedures.
- Provides initial patient assessment and comprehensive nursing assessment, identifying patient problems, reassessing nursing needs, and collaborating to develop the Hospice Interdisciplinary Team (IDT) plan of care.
- Teaches, supervises, and counsels the patient, family members and staff regarding patient's needs and nursing interventions, including possible other related problems of the patient at home.
- Works closely with other members of the IDT to ensure all patient and family care needs are being met.
- Complete documentation within agency guidelines and to meet regulatory requirements.
- Participate in the agencys orientation and in-service training programs for professional staff.
- Participate in agencys performance improvement activities and marketing and community programs as requested, to promote professional growth and understanding of hospice care.
- Demonstrate familiarity with policies of the agency and rules and regulations of State, Federal and governing bodies.
